Reciting due to noncompliance with submitting tracking sheets on a weekly basis. During the visit, staff #1 was observed to be wearing earbuds while supervising children. Staff #2 was observed to be looking at a cell phone while children were napping in room 5. Staff #3 was observed to be looking at a cell phone while supervising children in the infant room. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: 1. Children must be supervised at all times. 2. The legal entity will submit their supervision policy, which shall cover cell phone use, including ear buds, to DHS for review. Verified. 3. The legal entity will review the supervision policy with staff and document staff acknowledgement of the policy at the time of review. 4. Legal entity and members of management will conduct and document random supervision checks in classrooms (a minimum of 2 checks per classroom per week) to ensure staff are not on their phones/earbuds. During the visit, a smoke alarm was heard to beep intermittently, signaling the need for a new battery. The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Staff persons shall complete professional development in the topic of pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire. During the inspection, multiple ceiling tiles were observed to be sagging and water-logged. These ceiling tiles were located in the hallway leading to the classrooms and kitchen. Water spots were also observed in classroom 4. A rug in the back right hand side of the large playroom was observed to have curled edges, creating a potential tripping hazard. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Repair service has been contacted to address the roof leaks. Tiles are replaced as needed but the leak needs to be fixed on the roof. provider removed the rug from the space. Children in Room 5 were napping in the room with no source of natural or artificial light, making it difficult for Staff #2 to identify the children during the supervision check. Requirement: Rooms, hallways, stairways, outside steps, porches and ramps shall be lighted by artificial or natural light. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. It was observed in the 2's classroom, and in the lobby, that the ceiling vents are dusty (including the ceiling vents and wall vents close to the ceiling). It was observed that there are many water-stained ceiling tiles in the back hallway, young toddler classroom, and preschool classroom. Director previously requested landlord to maintenance the stained tiles. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Two's classroom and lobby ceiling vents were cleaned right away. Landlord has been contacted numerous times about leaking roof and stained ceiling tiles. It was observed in the 2's classroom, there is peeling paint with exposed plaster on the far wall, behind the table. CORRECTED ON SITE. It was observed in the lobby, to the right of the desk, there is peeling paint. CORRECTED ON SITE. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: Peeling paint was fixed in the two's classroom and the lobby during the inspection by covering with tape. Requirement: The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP. A child's record shall contain initial and subsequent health reports. Correction: Child 1 has a current health assessment on file. The facility will make sure an initial health assessment is submitted within the child's first 60 days of enrollment going forward. It was confirmed by staff person #2 that on 2/7/25, staff person #1 and #2 were arguing and yelling in front of children in the classroom. The daily activities in the three-year-old classroom were not promoting development of skills, social competence and self-esteem. Requirement: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background. Correction: Staff person #1 was let go. Director had a meeting with staff person #2. Director will visit the classrooms throughout the day to ensure daily activities are appropriate for all age groups. Inspection: Complaints- Legal Location. Staff person #1 possessed a stun gun (taser) within the child care center multiple times, and it was accessible to children. It was observed on 2/20/25, in the preschool classroom (down the left hallway) that a ceiling tile has water damage, and the ceiling vent is covered with dust in the back, left corner of the classroom (above the refrigerator). It was observed in the preschool classroom (last one down the back hallway) that the ceiling vents are covered with dust. It was observed in the older toddler classroom (last room down the left hallway) that the ceiling vent in the left corner of the room (above the circle time rug) is covered in dust. It was observed in room 10 (next to the infant room) that there was a light panel cover that had evidence of water damage, and dirty. CONTINUED NON-COMPLIANCE: On 3/25/25, It was observed that the ceiling vents were not cleaned, and are still covered with dust in the preschool classroom (front hallway-room 2), older toddler classroom (front hallway-room 1), and preschool classroom (back hallway-room8) It was observed that the water-damaged ceiling tiles were still not replaced in the preschool classroom (front hallway- room 2), and room 10 (next to the infant room). Also in room 10 there was a light panel cover that had evidence of water damage, and still dirty, that has not been cleaned or replaced. In addition: It was observed on 3/25/25, that the kitchen door was open, giving children access to a functional stove/oven. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The vents will be cleaned in both preschool classrooms and older toddler room.. The light panel cover will be replaced in room 10. The ceiling tiles will be replaced in the preschool room. A gate was added right away to the kitchen, for the stove to be inaccessible to children. Inspection: Unannounced Monitoring. It was observed in the infant room that bottles are hand washed, and there is not a commercial dishwasher on site. It was observed in the infant room that there are unlabeled bottles in the refrigerator and on the counter. Corrected on site. Requirement: Disposable nursers shall be used for infants unless bottles are provided by the parent or unless a commercial dishwasher is used by the facility. Disposable nursers and bottles shall be labeled with the child's name. Correction: All bottles are labeled. Parents will provide enough prepared bottles per day. Inspection: Renewal. The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. It was observed that staff person #3 diploma needs evaluated for equivalency to verify education prior to service at the facility. Requirement: A facility person's record shall include verification of child care experience, education and training prior to service at the facility. A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity. Correction: We will make sure verification of child care experience, prior to service will be documented, and transcripts will be translated/evaluated for equivalency. Inspection: Renewal.
