The individual file for staff person #1 does not include a written report of an initial health assessment. Requirement: A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. Correction: Staff #1 was spoken to by center director and explained that her health report was missing the checkmarks of the physician for her health report but that her TB test was verified. A medication log for child #5 has not been established by the operator for a prescription medication on site. Requirement: An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered. Correction: Center management contacted the mother of child #5 and explained that there was missing information for a rescue medication in the classroom and that the doctor's note and prescription would need to be provided. The health report on file for child #1 did not include a signature by a physician, physician's assistant or a CRNP. Requirement: A health report must be written and signed by a physician, physician's assistant or a CRNP. The signature must include the individual's professional title. Correction: Center management contacted the mother of child #1 and informed her that a physician's signature was missing on the health report and would be required. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. The parent letter pertaining to the facility's emergency plan does not include a method for facility persons to inform parents that the emergency has ended and to provide instruction as to how parents can safely be reunited with their children. Correction: TodayCare owner created a new family letter to be added to the Parent Handbook and Emergency Plan that includes the reunification of families and their children. Staff person #2 did not complete the required Pediatric First Aid and CPR training within 90 days of their hire date (see LIS sheet). The completed Pediatric First Aid and CPR training for staff person #2 is dated 3/27/25. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Correction: Staff will be required at hiring to acknowledge that they are responsible for completing the required CPR and First Aid training, even if Center Director is unable to schedule a training for them and without the former approved OCEDL approved codes to cover the cost. It was observed in the older toddler classroom a container of disinfecting cleaning wipes on a low cubby accessible to children. (This was corrected on site) Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: Center staff removed the cleaning wipes from the table and placed them in the locked cabinet about the counter space. Inspection: Renewal. It was observed in the infant classroom a Magic Eraser, SOS scrub pad, and a liquid in a soap like pump dispenser were not in an original labeled container or in a container that specifies the content. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces. Correction: The Magic Eraser and SOS pad were placed in a Ziploc bag and were labeled with their contents. It was observed in the preschool classroom two area rugs in the reading corner had smooth backing creating the area rugs to be slippery on the tiled floor causing a potential tripping and/or falling hazard. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Supervised at all times (3270.113(a))

It was observed in the older toddler and the younger toddler classrooms, sound machines being used at high volume levels that could potentially prevent staff from being able to properly hear, direct and assess toddlers while sleeping on nap mats. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: Center staff will ensure that each classroom is in ratio, including nap time, being able to see and hear the children at all times. Staff will rearrange classroom furniture to see children from all angles of the classroom. Center staffed removed one of the sound machines from the classroom in younger toddlers and staff in older toddlers, have turned down each of the three sound machines. Staff walked around the classroom practicing if they could hear the sounds a child may make etc... coughing, crying or stirring. Inspection: Allocated Unannounced Monitoring. It was observed in the older toddler classroom, Swiffer Wet pads, disinfecting cleaning wipes, and floor cleaner were stored in an unlocked cabinet under the sink accessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: Center staff were provided with a list for each classroom that describes where to locate cleaning supplies, emergency binders and OTC medications. Staff then walked through the classrooms to ensure that they knew where to find and put things away correctly. Staff were reminded to lock all cabinets. Staff person #1 did not complete mandated reporter training within 60 months of the previous training. The current training on file was completed on 11/24/24 and the previous training was completed on 9/30/19.
