It was observed that the fire drill log documented drills completed on 11/8/24 and 1/10/25, which lapses 60 days. Requirement: The Director or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that fire drills are conducted at least once every 60 days. Child # 2 immunization record does not include the 6-month Rotavirus vaccination. Child #5 immunization record does not include Hepatitis A vaccinations. See code sheet for date of birth and date of admission. Requirement: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. Correction: In reviewing Child #2's record, it appears that the child received the Rotavirus vaccination on 7/15/20 and 9/17/20. The third vaccination is due within 6 months of birth. The child's next appointment was during the 7 month of birth and therefore does not qualify for the third vaccination. The same is true with Child 5's records. Child # 5 received the Hepatitis A vaccination on 6/27/23 but was past the deadline age of 23 months and therefore did not qualify for the 2nd dose. When an immunization is missed, or rejected, a parent will be required to submit a written note, or documentation from the physician informing the facility of the reason for the lapse in immunizations. It was observed that child # 1 and 3 agreement forms were not reviewed, updated and signed by a parent once within a 6-month period. Child #1 lapsed 10 months. Child #3 lapsed 15 months. Requirement: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. Supervised at all times (3270.113(a))

It was observed in room 2 (2-year-olds) that a child was able to climb onto the changing table themselves, and sat on the changing table unsupervised. Staff #7 was directed to remove the child from the changing table. It was observed in room 4 (infants/young toddlers) that two children were asleep in the pack 'n' plays, out of sight and unsupervised, around the corner from where the two teachers were standing at the highchairs to feed the three young toddlers. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: Staff 7 was redirected and received disciplinary action. Staff 7 also received additional training from the Director regarding appropriate supervision of children. In room 4, pack and plays were rearranged to ensure no children are out of sight. It was observed in room 4 (infants/young toddlers) that there is a missing ceiling tile in the back, left corner of the sleep area, above the pack 'n' plays. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
