Observed a light in the left closet located in the Frogs 1 classroom which was sitting on top of the metal ceiling tile grid with a loose ceiling tile resting on top of the light. Observed visible dirt and dust along the baseboard trim on the wall in the Frogs 1 classroom. Observed the baseboard strip on the wall by the sink in the Chinchilla's classroom was pulling away from the wall. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Adult scissors were immediately placed outside the reach of children in a locked drawer and the utility knife was removed from the Hedgehogs classroom. A Cricut machine was immediately removed from the Polar Bears classroom. Food and crumbs were immediately cleaned up from the sink area and the step stool in the Penguins classroom. Ceiling light was turned off and the ceiling tile was repaired. In addition, baseboards were dusted and cleaned in the Frogs 1 classroom. Baseboard strip was repaired in the Chinchillas classroom.     The pediatric first aid and CPR for staff person #1, #2, #3 was not renewed on or before the expiration of the previous certification. The current training on file was dated 04/13/2024 with the previous training dated 03/24/2022 with an expiration date of 03/2024. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Correction: Typically we do pediatric first aid twice a year to ensure everyone meets the DHS required standard. Follow Me had three teachers who were unable to attend the scheduled pediatric first aid class due to a family emergency. They made up the class within 30 days of the scheduled class. When a child is awake and no longer resting on their mat, naptime ratios no longer apply. Requirement: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30. The staff:child ratio while toddlers and preschoolers are napping is one staff person to 10 young toddlers, one staff person to 12 older toddlers, one staff person to 20 preschool children. Correction: Follow Me Christian will maintain maximum group ratios while children are both awake and napping. If a child is awake and no longer resting on their cot, the teacher will notify the director so that an additional staff person can join the classroom to maintain ratios. During the inspection visit, another staff person joined the classroom for the remainder of nap time so that ratios were compliant. Inspection: Renewal. From their position in the bathroom, staff person #1 was able to supervise 3 children napping nearby but was unable to supervise the remaining 6 children napping in the classroom. 1 child was behind a shelf, and 5 children were on the other side of the room which were blocked from view by the wall in the center of the room. There were 4 older toddlers and 6 preschool age children with 1 staff person in the classroom during naptime. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: Children on the facility premises shall be supervised by a staff person at all times. With this particular classroom, Follow Me Christian will put an extra person in the classroom over naptime so that blind spots will not be created especially if assisting a child in the bathroom.
