Requirement: The facility shall have an emergency plan that provides for accommodations of infants and toddlers, children with disabilities, and children with chronic medical conditions. Correction: I added a section into my Emergency Plan and Parent Letter addressing accommodations for infants, toddlers, children with disabilities, and or chronic medical conditions. The updated emergency plan has been printed and filed. The parent letter has been updated, printed and displayed on the parent information boards. Inspection: Renewal. Correction: I rewrote my Shaken Baby/Maltreatment Policy, made copies and put in Policy Files for future use and review. Inspection: Renewal. The emergency plan did not include a method of informing parents that an emergency has ended and how parents can safely reunite with their children. CORRECTED ON-SITE. Requirement: The facility shall have an emergency plan that provides for a method for facility persons to inform parents that the emergency has ended and to provide instruction as to how parents can safely be reunited with their children. Correction: I added a section into my Emergency Plan and Parent Letter addressing how families will be contacted at the time of event, after we were safe and can contact them, after the event and give instructions to reunite with their children. The updated emergency plan has been printed and filed. The parent letter has been updated, printed and displayed on the parent information boards. Inspection: Renewal. Updated toddler/preschool health report: every 12 months (3290.131(b)(2))

The file for child #2, included health reports which were not updated at least once every 12 months for an older toddler or preschool child (see LIS code sheet). The file included a current health report dated 01/29/2024 with an initial health report dated 12/22/2022. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. Correction: All families have been informed that they must bring in and schedule yearly physical/well check using PA state forms and having doctors sign this form along with their office forms. The letter provided to the parent of each enrolled child explaining the emergency procedures did not include information regarding accommodations for infants, toddlers, children with disabilities and children with chronic medical conditions as required in subsection (a). Requirement: The operator shall provide to the parent of each enrolled child a letter explaining the emergency procedures described in subsection (a). The operator shall also provide to the parent of each enrolled child a letter explaining any subsequent update to the plan. Correction: New letter created with the paragraph of accommodations and given to families. This was mentioned in violation 1 and special needs file created so in the future we can add any other child's special accommodations to that. Inspection: Renewal. The annual emergency drill was not conducted timely, the most recent drill was documented on 06/10/2024 with the previous drill documented on 01/24/2023. Requirement: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility. Correction: I plan to document, file and conduct an annual emergency drill in January putting the drill at 8 months. I have also placed this on the laminated list of yearly required drills, reviews, renewals. I will need to then time the following years drill for December or sooner to be within a years time. The operator did not review the emergency plan annually and update the plan as needed. The last documented review of the emergency plan was dated 09/27/2022. Requirement: The operator shall review the emergency plan at least annually and update the plan as needed. Each review and update of the emergency plan shall be documented in writing and kept on file at the facility. Correction: I have created a laminated yearly important re-do/review list and placed it in my calendar under the months it should occur. Since I've rewritten the entire emergency plan, the next review will be October 2025 or earlier so that it is reviewed within a years' time. Operator or designated staff responsibility (3290.95(b))

The operator did not ensure a manual test of the fire detection system is conducted at least once every 30 days. The manual test conducted in the following periods exceeded 30 days: 12/01/2023 - 01/02/2024, 02/01/2024 - 03/05/2024, 05/01/2024 - 06/03/2024, 08/01/2024 - 09/03/2024. Requirement: The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in § 3290.95(a) are met. The file for child #1 included a Well Child Report dated 07/23/2024 which did not include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease. The file for child #2 included a Preventative Exam Summary dated 01/29/2024 which did not include a statement that the child is able to participate in child care an appears to be free from contagious or communicable disease. Requirement: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease. Correction: Each family was given a new PA state health report sheet, given directions to sign every section, answer questions and have doctor sign or doctor's stamp added to this form. The file included a current health report dated 07/23/2024, an immunization record dated 05/25/2023 and an initial health report dated 05/10/2022. Requirement: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. The emergency contact information and financial agreement for child #1, #2, #3, and #5 is not reviewed by a parent at least once every 6 months. The most recent review was completed on 08/01/2024 with the previous reviews completed in 08/2023. The forms are reviewed annually in lieu of every 6 months. Requirement: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. Correction: Operator thought and has always done agreements once a year- all previous inspectors said yearly agreements. However, I have now added a review 6-month form to client's folder for emergency contacts and financial or contract review. Inspection: Renewal. Parent home/work address, phone (3290.124(b)(3))

The emergency contact information for child #1 did not include the work address and telephone number of the enrolling parent, Requirement: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent. Correction: This was taken care of within several days of the inspection. Operator showed the mom, and she filled out the information immediately. Inspection: Renewal.
