Observed charging cords which were plugged into an outlet but not plugged into a device lying on the floor in the PreK classroom. *Corrected at the time of the inspection. A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency was not posted in the infant classroom. *Corrected at the time of the inspection. The file for child #1 included a health report which was not updated within 12 months of the previous health report for an older toddler or preschool child (see LIS code sheet); the current health report on file was dated 11/13/2024 with the previous health report dated 10/18/2023. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. During boarding and exiting (3270.177(b))

Upon returning to the facility from transporting 2 younger school age children from school, observed staff person #2 open the bus door for the children to exit the vehicle. Staff person #2 exited the vehicle behind the children and closed the vehicle door. An adult was not observed outside the vehicle while the children exited. Requirement: Children shall be supervised during boarding and exiting vehicles by an adult who remains outside the vehicle. Correction: An adult will remain outside the vehicle at all times to supervise children during boarding and while exiting the vehicle. Inspection: Allocated Unannounced Monitoring. Age and Training (3270.31(e)(4)(i))

Staff person #1 has not completed Pediatric first aid and CPR training on or before expiration of the most current certification. This is evidenced by the previously documented Pediatric first aid and CPR training on file expiring 04/2025. Documentation of updated Pediatric first aid and CPR training is not on file Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Until such time as the required training has been completed, staff person #1must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #1, staff person #1 may not work in a child-care position at the facility. Observed the hot water temperature of the sinks located in the young toddler classroom, young todder bathroom, school age classroom, and school age bathroom reached a temperature of 111 - 112° F. Requirement: Hot water temperature, in areas accessible to children, may not exceed 110° F. Correction: Hot water temperature was adjusted so it will not exceed more than 110 degrees Fahrenheit. Small Toys and Objects (3270.103)

Observed small stones in the play kitchen sink located in the toddler outdoor play space which were less than 1 inch in diameter and accessible to children who are still placing objects in their mouths. The stones were accessible through a gap at the bottom of the fence along the concrete area in the play space. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: Small stones were removed from the play kitchen area. Lattices were added to the bottom of the fence, so the rocks are not accessible to the children. The director or designated staff person did not ensure a manual test of the fire detection system was completed within 30 days of the previous test. Disposable nursers or parents (3270.166(3))

Reusable bottles were observed in a drying rack located in the infant room. According to staff, the bottles provided by the parents are hand washed and reused. Staff stated that some parents take them home each day and others are returned to the parent on Fridays. The facility is not washing the bottles with a commercial dishwasher between uses. Requirement: Disposable nursers shall be used for infants unless bottles are provided by the parent or unless a commercial dishwasher is used by the facility. Correction: Parents will be providing the number of bottles their child needs for 1 day. Parents will take home all bottles wash and sanitize and bring them back the next day. The file for child #2 included a CSR completed on 03/01/2024 with no subsequent CSR on file. The file for child #3 included a CSR completed on 10/21/2024 with the previous one completed on 03/18/2024. The file for child #4 included a CSR completed in 02/2024 with no subsequent CSR on file. Requirement: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. General Health and Safety/Infant sleep position (3270.21/3270.119)

Observed Pack N Plays in the infant classroom with regular size crib sheets on the mattresses in lieu of sheets intended for a Pack N Play mattress. The sheets were too big for the mattress and created access material which creates a potential suffocation hazard. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.
