It was observed that the facility tested smoke detectors on 12/19/24 and then again on 1/21/25, which is greater than the allowable 30 days between testing. (33 days lapsed). Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Correction: Facility smoke detectors were tested during inspection and will continue to be tested every 25 days to ensure that it is meeting the 30 day maximum requirement. Inspection: Renewal. Health and Safety Training - Pediatric First Aid and CPR (3270.31(f)(10))

Upon inspection of facility personnel files, it was found that facility person # 21 has not completed Pediatric first aid and CPR training on or before the 90- day pre-service training timeframe. This is evidenced by the documented pediatric first aid and CPR training on file was a date of 11.4.25 (92 Days). Requirement: Staff persons shall complete professional development in the topic of pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire. It was observed that medication in the cottage young toddler room did not include written consent for administration. (CORRECTED ON-SITE) Requirement: A parent shall provide written consent for administration of medication or a special diet. Correction: The medication was sent home with child's parent and will be accepted back only with the proper prescription and written consent for administration. Inspection: Renewal. Clean and good repair (3270.102(a))

It was observed that a soft cushion in the D1 room had frayed edges. ( CORRECTED ON-SITE) Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Building Surfaces (3270.76)

It was observed that the cottage's toddler deck had metal flashing that was in need of repair. (CORRECTED ON-SITE) Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: All weather adhesive tape was used to cover the metal flashing until a more permanent repair can be made. Inspection: Renewal. It was observed that the first aid kit in the D1 room did not include soap. (CORRECTED ON SITE) Requirement: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves. Correction: We will ensure that all classroom first aid kits contain all of the required items including soap at all times. Inspection: Renewal. Staff evaluations (3270.34(a)(6))

Upon inspection of facility personnel files, Staff evaluations were not completed on a regular basis at a minimum annually. Facility person # 1 had an evaluation dated 9.25.24 and again on 10.17.25. Facility person #2 had an evaluation dated 9.30.24 and again on 10.20.25. Facility person #3 had an evaluation dated 9.26.24 and again on 10.17.25. Facility person #6 had an evaluation dated 9.18. 24 and again on 10.17.25. Facility persons #15 and #19 had an evaluation dated 9.23.24 and again on 10.21.25 and 10.20.25. Requirement: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months. Correction: We will ensure that all staff have an evaluation on or before their expiration date (12 months from previous evaluation). Inspection: Renewal. Upon inspection of facility personnel files, it was found that for facility person # 10 contained a NSOR verification that was dated 1.14.20 (expired on 1.14.2025) was updated on 1.31.25. The file for facility person #15 contained a Child Abuse clearance that was dated 9.3.20 (expired on 9.3.2025) was updated on 10.11.25. The file for facility person #16 contained a Mandated Reporter training that was dated 1.24.20 (expired on 1.24.2025) was updated on 2.06.25. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Correction: We will ensure that staff renew all clearances/mandated reporter training on or before the date of expiration. Inspection: Renewal. In the Cottage building -' Infant room', the protective surfacing along the wooden half-exit door of the indoor play space was previously repaired with duct tape and was observed to be in disrepair with several frayed pieces of tape hanging along the edges and in need of repair. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The frayed duct tape was immediately removed from the door surface to eliminate any potential hazards. This will prevent children from coming into contact with loose or damaged materials. We will replace the duct tape with a more durable protective surface material appropriate for long-term use on the door which will have a child-safe, soft protective edging that is designed for heavy use and will not fray or degrade quickly. Reports of accidents, injuries and illnesses. Original report - parent same day Copy - facility accident file. Copy - child's file (3270.182(7))

Facility person #1 did not provide a parent an injury report on the day of the incident dated 9.13.24. The Incident report was given to the parent on Tuesday, 9/17/24. Requirement: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file. Correction: PLEASE BE ADVISED THAT HANSEL & GRETEL ELC respectfully disagrees with the issuance of this citation related to this incident. The child in care suffered a seizure while playing out on the playground, and. as a result of the seizure, the child fell from a higher platform which was a part of outdoor play equipment. The child¿s parent is an employee of Hansel & Gretel and immediately came to the playground. EMTs were called and, upon their quick arrival, EMT personnel stressed the urgency in transporting the child to the hospital due to the seizure, but also because they were unsure if the child had suffered a head or neck injury due to the fall the child sustained. The child was hurriedly placed in the ambulance, with the child¿s mother accompanying her son, and rushed to the hospital. Hansel & Gretel certainly could not have stopped the EMTs from transporting the child, possibly interfering with the child receiving the necessary immediate medical care, to have the parent sign said incident report. As per the purpose listed in the PA Chapter 3270, the regulations are to ¿facilitate the safe and healthful care of a child in a child care center with a purpose to provide standards to aid in protecting the health and safety of children¿To stop the child and accompanying parent from immediately being rushed to the hospital certainly would not have facilitated the safe and healthful care of the child nor would it have protected the health and safety of the child. In fact, stopping the EMTs from rushing the child and parent to the hospital, would have been done the exact opposite of what the child care regulations are meant to do. Thus, Hansel & Gretel made the decision that the child¿s well being took precedent and made the decision not to interrupt the child receiving care, but rather to provide the incident report to the parent as soon as possible. It is Hansel and Gretel ELC¿s belief that this citation should be removed as it found itself in an almost impossible situation and chose to take action that was in the best interest of the child. However, Hansel & Gretel ELC, in keeping with Licensing¿s directive, respectfully submits this Plan of Correction. Issue Identified: On September 13, 2024, a staff member, who is also the parent of the child involved in the incident, left the facility with her child in the ambulance following a fall and seizure. As a result, the injury report could not be provided to her on the same day, and it was instead given on September 17, 2024 when she returned to work. 1. Explanation and Apology to the Parent: o Immediate Communication: As the child¿s mother left with the child in the ambulance, an explanation for the delay in providing the injury report was communicated to her as soon as possible. o Apology for the Delay: A formal apology was offered for the delay, clarifying that the report was not issued on the day of the incident due to the unique situation where the parent and staff member was not present to receive it. o The director was in communication with the Mother in order to get updates on his condition ¿ 3 times over the weekend. (Outcome: No injuries due to fall. No memory of or lasting effects from the seizure. Child returned to full activity on September 17, 2024.) 2. Completion and Delivery of Injury Report: o The injury report was completed immediately after the incident and was delivered to the parent on September 17, 2024, as soon as th
