Staff persons #1 and #2, lapsed supervision on 12/18/25 when child #1 exited the classroom without supervision, unaware that child #1 was no longer in the classroom. Staff person #1 and #3 observed child #1 exiting the bathroom to return to the classroom. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: Management reviewed the supervision policy with all staff members. We also reviewed the potty policy with students to ensure understanding of asking before going across to the bathroom. Inspection: Self-Reported Non Compliance. Staff person # 1 and 2 did not obtain the Child Abuse Clearance prior to the first day caring for children. Staff person #1's Child Abuse Clearance is dated 7/5/25. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Correction: New staff will have all clearances for their file, before caring for children. Staff Data worksheets will be completed prior to start date. Inspection: Renewal. It was observed that children # 1-10's files contained the original financial agreements. Requirement: A parent shall receive the original agreement. The facility shall retain a copy of the agreement. A child's record shall contain a copy of the initial agreement and subsequent written agreements between the parent and the operator. The parent receives the original agreement. Correction: Children #1-10's original agreements were given to families.
