Documentation showed a fire system test conducted on 4/10/26 and again on 5/11/26, exceeding the requirement that fire system tests be conducted every 30 days. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met. Small Toys and Objects (3290.103)

During the inspection, posters were observed to be attached to the wall using flat push pins. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: Provider removed all pushpins within reach of the children. Provider will replace the pushpins with sticky tabs to hang the wall decor. Inspection: Renewal. Comply with CPSL/CPSL information (3290.32(a)/3290.192(3))

Documentation showed that staff #1 had a PA state police clearance dated 1/14/26 with a previous PA state police clearance dated 10/21/20, exceeding the requirement that clearance be renewed every 60 months. Documentation showed that staff #1 had a child abuse clearance dated 1/16/26 with a previous child abuse clearance dated 11/18/20, exceeding the requirement that clearance be renewed every 60 months. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 may not work in a child care position at the facility. Correction: The Provider obtained the state police clearance on 1/14/26 and the child abuse clearance on 1/16/26. Documentation showed Staff #1 completed CPR training on 6/1/23 and again on 10/9/25, exceeding the requirement that CPR training be completed on or before the expiration date. Requirement: Competence is the completion of training by a professional in the field of first-aid and CPR. All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. No peeling paint or plaster (3290.75(a))

It was observed that there is a hole in the chalkboard wall, with exposed drywall. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Updated toddler/preschool health report: every 12 months (3290.131(b)(2))

Child #6's file does not contain an updated health assessment within 12 months. The last child health report in file is dated 4/26/24. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. Correction: I will make sure to have a current physical dated within 12 months in the children's file. Child #6 has a doctors appointment scheduled for 6/3/25. Inspection: Renewal. A child's record must contain a copy of the initial agreement and subsequent written agreements between the parent and the operator. The parent receives the original agreement. Supervised at all times (3290.113(a))

It was observed that staff person #1 answered the door, for Cert. Rep. to enter, with two children, two more children were in the child care space unsupervised. It was observed that staff person #1 walked out of the room, to the kitchen multiple times, leaving three or four children unsupervised. It was observed that child #1 walked out of the room multiple times, without supervision. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. The requirement for supervision on and off the facility premises includes compliance with the staff:child ratio requirements in §§ 3290.51-3290.52 (relating to maximum number of children; and staff:child ratio). Correction: I will use gates to prevent children from leaving the daycare area. I will use a video monitoring system when I need to leave the room and can't physically see the children - prepping meals and using the restroom. Inspection: Renewal. It was observed that there is not 2 feet of space on three sides of the pack and plays used for rest times. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. It was observed that there were stacks of paper and clutter on the table, and on top of the printer. It was observed that there was debris on the carpets and floor. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards. Correction: All surfaces will be free of clutter. Floors and carpets will be swept and mopped each night before care. Any spills or mess that occur during care will be cleaned up immediately. First-aid kit does not contain disposable, nonporous gloves. Requirement: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves. Correction: I will make sure all required items, including disposable and nonporous gloves, are in my first aid kit at all times. Inspection: Renewal. The emergency plan does not include information on a method for facility persons to contact parents when an emergency arises, ends or instructions for how to reunite parents with children. Requirement: The facility shall have an emergency plan that provides for a method for facility persons to contact parents as soon as reasonably possible when an emergency situation arises. The facility shall have an emergency plan that provides for a method for facility persons to inform parents that the emergency has ended and to provide instruction as to how parents can safely be reunited with their children. Correction: I will add the required verbiage to my emergency plan so that parents know how to pick up their children after an emergency, and that parents will be contacted when an emergency begins and ends.
