The play equipment was not maintained in a safe or sanitary condition for the children to use in that [there was a green substance on the inside of the white basketball thing and the railing to the steps of the wooden jungle gym was deteriorating. ]. CCF Handbook, Section 3.12, B and F Sanitation and Equipment [SR] Comments: During the previous inspection, the Licensing Specialist observed that the play equipment was not maintained in a safe or sanitary condition for the children to use in that the wood on both sides of the wooden ladder and the yellow chain link ladder were damaged and had a green substance. The wooden piece on the left of the wooden ladder has a green substance, and the wooden piece on the right of the yellow chain ladder has jagged edges and pieces that are peeling. This area is the wooden structure is in the back of the building, furthest away from the front entrance. During the previous inspection, the Licensing Specialist observed that the play structure that is white, that stands between the playground near the front entrance and the playground near back area of the facility had a green and dark colored substance on the outside and inside of it. During the reinspection, Licensing Specialist observed that the the wood and green substance was replaced and removed. The white play structure still had green and dark colored substance on the inside of it. This is being marked non-compliant. During the reinspection, Licensing Specialist observed on the wooden jungle gym the wooden sides of the stairs was deteriorating and had sharp pieces. The director was given until February 2, 2026, to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, CCF Handbook, Section 3.12, B and F: B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. F. All equipment used in the outdoor play area must be constructed and maintained according to manufacturers recommendations and allow for water drainage. CCF Handbook, Section 3.1, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the inspection, the Licensing Specialist observed areas of the facility that were not be in good repair in that there were wires exposed and accessible under the dashboard in front the passenger seat in the vehicle. During the inspection, the Licensing Specialist observed that the toilet seat in the VPK classroom was not in good repair in that the seat was peeling, The Operator has been given until 2/9/2026 to come into compliance with this standard. A safety deficiency having a low potential for harm to the children in care was observed. CCF Handbook, Section 3.1 (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the inspection, Licensing Specialist observed on the big playground the pathway that is by the building there was a hose sitting out and not hooked up to the faucet. Also there was some open miracle sitting up by the kitchen door. No one was on the playground at the time the specialist was inspecting the playground. The assistant director brought this standard back in compliance at the time of inspection by putting both items in a secure location that the child does not have access to. A fire hazard, [dust build up on the bathroom vents], was observed in the facility. CCF Handbook, Section 3.1, B (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the inspection, Licensing Specialist observed a buildup of dust on the vents in the all the bathrooms. The staff member brought this standard back in compliance at the time of inspection by cleaning the vents.
