41. Immunization Records CCF Handbook, Section 7.1 (41-02)

The Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)]. CCF Handbook, Section 7.1 Record Keeping [SR] Comments: During the time of the inspection, Director advised the Licensing Specialist that there are currently 19 children enrolled. During the time of the inspection, the Licensing Specialist monitored 19 of the 19 childrens records. During the inspection, the Licensing Specialist observed two children who had expired Immunization forms. The director was given until April 7, 2025, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 7.1 Immunization Records: The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C- 22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. Medication which had expired or is no longer being administered was not discarded or returned to the custodial parent or legal guardian. CCF Handbook, Section 6.5, H Health Requirements [SR] Comments: During the inspection, Licensing Specialist observed that there was medication expired at the time of the inspection. The medication, an EpiPen, was given to the custodial parent or legal guardian on 10/17/2024. The medication, an EpiPen, expired on 10/2024. This standard was brought back into compliance at the time of the inspection in that the medication was returned to the custodial parent or legal guardian at the time of the inspection. An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Physical Environment [SR] Comments: During the inspection, the Licensing Specialist observed that the in the VPK classroom there was paint chipping at the bottom of the television stand that was on the left side of the room. This standard has been given a due date of April 7, 2025 to come back into compliance. During the inspection, the Licensing Specialist provided technical assistance. CCF Handbook, Section 3.1, A: General Health and Safety Requirements: All childcare facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. The facility used electronic media time with children younger than 2 years of age. CCF Handbook, Section 2.6, C (Section 2.1 Health and Safety, Page 3) General Requirements [SR] Comments: 9-6: During the inspection, Licensing Specialist observed that there were one year olds watch TV (electronic media) while they were eating lunch. The TV was turned off. This is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 2.6, C Do not include the use of electronic media for children under two years of age. Electronic media may only be used for educational purposes or physical activity for children 2 years of age and older for no more than 1 to 2 hours per day.
