Child care personnel failed to share the accident/incident form with the custodial parent/ legal guardian or the individual authorized to pick up child on the day that the incident occurred. CCF Handbook, Section 6.4, B & D Health Requirements [SR] Comments: 39-4: During the inspection, the director stated that the teacher write an incident report for child 1. An incident report was not written for child 2 because she did not see any hair cut on child 2. At the end of the day when child 1 went into a different classroom, the teacher did not move the incident report to child 1s closing class, so the parent did not get the incident report at the end of the day. The mom texted the director asking about what had happened the child 1s shirt. The director stated what had occurred and said that the incident report is waiting for her to sign the next morning. When the mom came in the next day she signed the incident report, however she dated her signature for the date of the incident instead of the date signed. Supervision of children in the [three year old] group was inadequate in that [a number of children were not being safe with scissors]. CCF Handbook, Section 2.4 General Requirements [SR] Comments: 4-01: During the inspection, Licensing Specialist observed the video footage from 2/18/2026 of the three-year-old room. At the 9 second mark child 2 is cutting with the scissors above child 1s head and then child 1 cuts above child 2s head. Child 2 turned to the child next to him (in a green shirt) and cuts with his scissors at the child, the child in the green shirt pushes the scissors away. Child 2 turns back toward Child 1 and reaches to cut above Child 1s head and the child in the green shirt reaches above child 2s head and cuts with his scissors. The children stop and the teacher is seen by the table passing out more paper. The teacher is seen assisting a child with scissors. At the 1 minute 54 second mark, Child 2 turns toward child 1 and cuts at child 1s shirt. Child 1 looks down and touches his shirt. Child 2 cut at Child 1s shirt again and child 1 pushes child 2s scissors away. Child 1 is seen looking at his shirt. At the 3 minute 22 second mark, a child at table 2 in a blue shirt reaches behind him to cut at Child 2s shirt. At the 3 minute 48 second mark, Child 2 cuts at child 1s shirt again. At the other table, four children are cutting at each others hair. The teacher looks over and says something and the children stop. At the 4 minute 58 second mark, two children at table 2 (one in blue and one in white) are cutting at each other and cutting each others scissors. They stop at the 6 minute 4 second mark. At the 6 minute 15 second mark, a child with longer hair at table 2 appears to be cutting at his own fingers and then appears to cut at his own hair. At the 6 minute 57 second mark, the child in the black shirt at table 2 appears to be cutting at his own hair. At the 7 minute 24 second mark, Child 2 grabs Child 1s paper, child 1 pulls it back and child 1 cuts at child 2s shirt, then child 2 cuts at child 1s shirt. At the 7 minute 43 second mark teacher took away child 2s scissors. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 2.4 A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. C. Supervision standards apply at all times away from the child care facility, including during field trips, outdoor play, and when picking up or dropping off children at designated locations, such as bus stops, school, or a childs home. For school-age children, personnel must know where the children are and what they are doing at all times and capable of responding to emergencies including when children are separated from their group. Please note, you will only receive that notification for the staff that are on your facility/home roster. Please keep your roster current in the Clearinghouse to avoid confusion on expiration dates, or unwanted notifications. 45-4: During the inspection, Licensing Specialist observed 2 employee did not have documentation that the 5 year employment history was verified. CCF Handbook, Section 4.7, D. 1 and 2 Comments: During the time of inspection, Licensing Specialist observed the director responsible for daily operations was not at the facility. Licensing Specialist reviewed timesheets for director responsible and observed the last day the director clocked in was 10/07/2024. Staff advised the director was on leave due to damages at home from Hurricane Helene and Hurricane Milton. During the time of inspection, Licensing Specialist provided technical assistance; CCF Handbook Section 4.7 Director Credential: Section 402.305(2)(g), F.S., requires a child care facility to have a credentialed director.
