32. Outdoor Equipment CCF Handbook, Section 3.12 (32-04)

Inspections were not conducted and documented monthly of all supports, above and below the ground, all connectors, and moving parts. CCF Handbook, Section 3.12, B Comments: 32-4: During the inspection, Licensing Specialist observed that the physical environment log was not conducted monthly, in that the last entry documents was on 1/2/2026. During the inspection, Licensing Specialist provided technical assistance, Section 3.12, B All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. TRAINING Compliance Comments During the time of inspection, the Owner/Operator/Director advised the Licensing Specialist that there 10 employees. During the time of the inspection, the Licensing Specialist monitored 10 of the 10 employee records. REMINDER: In-service training for 2025/2026 fiscal year must be completed before July 1, 2026. The Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)]. CCF Handbook, Section 7.1 Comments: 41-2: During the inspection, Licensing Specialist observed 2 childrens files had expired immunizations. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 7.1 The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C- 22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Child care personnel including volunteers who work 10 hours or more per month did not complete the 40 hour Introductory Child Care Training requirement. CCF Handbook Section 4.2.1 Comments: During the time of inspection, the Owner/Operator/Director advised the Licensing Specialist that there 10 employees. During the time of the inspection, the Licensing Specialist monitored 10 of the 10 employee records. Counselor reviewed 10 hours of annual in-service, completed, for all child care personnel for the 2024/2025 fiscal year. The facility did not have documentation to show completion of a Department approved five-hour early literacy and language development course for child care personnel within 12 months of date of employment in child care industry and/or the early literacy course documentation was not uploaded in the Florida Pathways/Registry. CCF Handbook, Section 4.2.2 and 4.5, C Comments: 33-06: During the inspection, Licensing Specialist observed 1 child care personnel did not complete the DCF 5 hour early literacy training within 12 months of their child care industry date. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 4.2.2 and 4.5, C Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. Early literacy training must be completed within 12 months of date of employment in the child care industry. Proof of completion may be documented on a certificate of course completion, classroom transcript, or diploma. Child care personnel must complete one of the following: A. One of the Departments online literacy courses available on the Departments website. B. One of the Departments approved literacy courses. A list of these courses can be obtained from the Departments website. (No additional courses will be approved by the Department.) C.
