CCF Handbook, Section 6.5, A and 2.5.3, C & D (Section 2.1 Health and Safety, Page 3) Health Requirements [SR] Comments: During the inspection, Licensing Specialist observed that the medication forms for diaper ointment cream (Parent Choice Vitamin A&D Ointment) did not state how much to apply on the child and when to apply. It did not state what to look for. The director brought this standard back in compliance at the time of inspection by having the parent fill out the form. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 6.5 A. Medication. Child care programs are not required to give medication; however, if a program chooses to do so, it must comply with the following requirements: A. The child care program must have written authorization from the custodial parent or legal guardian to give prescription and non- prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and contain the childs name; the name of the medication to be given; and date, time and amount of the correct dosage to be given. Prescription and non- prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given. The child care provider must never administer a medication that is prescribed for one child to another child. RECORD KEEPING 41. The door on the right side of the classroom that leads out to the playground the drywall is peeling off and is exposing the board underneath. There is a rip in the red couch. In the bathroom there is a hole in the wall across from the toilet. In the older 2 year old room in the hallway the blue wall on the left the paint is peeling. The boarder at the bottom of the wall in circle time the paint is peeling. In the bathroom the wall across from the toilet the paint is peeling. In the younger 2 year old room there is a hole in the wall by the door leading into the 3 year old room. In the 3 year old room the shelf that is by the infant room the back of the shelf is peeling. In the infant room and the cafeteria the seats that are built into the tables were missing the safety restraints. In the cafeteria there is a hole in the boarder at the bottom of the wall by the bathroom door. On the chevy bus the seats are ripped by the seat buckle and exposing the foam.
