Additional documentation describing the symptoms that would require the prescription and/or non-prescription medication used on an as needed basis to be administered was not on file. CCF Handbook, Section 6.5, A and 2.5.3, C & D (Section 2.1 Health and Safety, Page 3) Health Requirements [SR] Comments: During the inspection, Licensing Specialist observed that the medication forms for diaper ointment cream (Destin, Boudreauxs Butt Paste, Caldesene, Aquaphor, Parents Choice, Triple Paste, A*D ointment, Advance healing ointment) did not state time and amount of the correct dosage to be given. Non- prescription medications did not provide additional documentation on the authorization form to describe symptoms that would require the medication to be given. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 6.5 A. Medication. Child care programs are not required to give medication; however, if a program chooses to do so, it must comply with the following requirements: A. The child care program must have written authorization from the custodial parent or legal guardian to give prescription and non- prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and contain the childs name; the name of the medication to be given; and date, time and amount of the correct dosage to be given. Prescription and non- prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given. The child care provider must never administer a medication that is prescribed for one child to another child. RECORD KEEPING 41. Child care personnel did not complete preservice training requirements prior to unsupervised contact with or care of children or, if supervised, within 90 days of beginning employment or service with the provider. (Section 3.1 and 3.2 Training Requirements, Page 7) [SR] Comments: During the time of inspection, the Owner/Operator/Director advised the Licensing Specialist that there 17 employees. During the time of the inspection, the Licensing Specialist monitored 17 of the 17 employee records. When the Licensing Specialist was reviewing the files the licensing specialist observed three personnel that did not complete their School Readiness training within the 90 days of beginning employment or service with the provider. The director was given until August 1, 2026, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, School Readiness Program Health and Safety Standards Handbook Section 3.1 Preservice. Child care personnel must successfully complete the preservice training coursework described in this section, as applicable. All child care personnel must complete these preservice training requirements within 90 days of initial employment with any School Readiness provider. This timeframe does not start over if personnel change employment to another School Readiness provider within this initial 90 days. General Requirements [SR] Comments: The ratio was non-compliant: 2:14 - 1(Infant), 4(1-year old's), 1(2-year old), 3(3-year old's), 1(4-year old) and 4 School Age 2 other personnel Corrected Ratio: 2:5 - 1(Infant) and 4(1-year old's) 1:9 - 1(2-year old), 3(3-year old's), 1(4-year old) and 4 School Age 1 other personnel During the inspection, Licensing Specialist observed 14 children with 2 adults in the classroom. The operator brought this standard back in compliance by taking the infant and the 1 year old's to another room and having another personnel that was at the facility come in the classroom with her. A napping or sleeping infant who is not capable of rolling over was not positioned on his/her back and on a firm surface, or was swaddled, and the provider did not have written authorization for an alternate sleep position from a physician in the childs record. CCF Handbook, Section 3.6.4, B (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: 20-4: During the inspection, Licensing Specialist observed that there was one infant sleeping, swaddled, with their arms restrained. During the inspection, staff took the infant out of the swaddle. This is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 3.6.4, B When napping or sleeping, young infants who are not able to roll over must be positioned on their backs and on a firm surface to reduce the risk of Sudden Infant Death Syndrome, unless an alternate position is authorized by a physician. Sleep sacks that fit according to manufacturers recommendations, do not restrict the infants arms, and will not slide up around the infants face may be used for the comfort of the sleeping infant; however, swaddling shall not be used unless authorized in writing by the childs physician. Written documentation of a physicians authorization must be on file at the facility. Documentation must include the childs name, childs date of birth, description of sleep position required, instructions for the use of any equipment needed, and length of time authorization is valid. 21. A vehicle(s) used to transport children did not have an annual inspection by a mechanic to ensure it was in proper working order. The facility's driver did not have [ An annual physical examination which grants medical approval to drive]. Driver 1s license will expire on 11/9/2026, the CPR and First Aid certificate is valid until 3/31/2026 and the driver 1 has completed the transportation training. 6-1: During the inspection, Licensing Specialist observed that that the 1 driver did not have a drivers physical conducted annually, in that the physical was conducted on 5/14/2024. The facility did not have a current and approved annual fire safety inspection by the local fire authority. CCF Handbook, Section 3.8.2, A (Section 2.1 - Health and Safety, Page 3) Physical Environment [SR] Comments: The last fire drill was conducted on 2/4/2025. The fire extinguisher certification(s) are valid until 5/2025. 23-2: During the inspection, Licensing Specialist observed that the facility did not have an approved fire inspection, in that the last approved fire inspection was conducted on 2/14/2024. A fire inspection was conducted on 2/24/2025, however the fire inspection had a non-compliance. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 3.8.2, A Unless statutorily exempted, all child care facilities must conform to state standards adopted by the State Fire Marshal, Chapter 69A-36, F.A.C., Uniform Standards for Life Safety and Fire Prevention in Child Care Facilities. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority. Food provided by parents/guardians was not properly stored and handled in a sanitary manner to prevent contamination or spoilage. CCF Handbook, Section 3.9.5, E Food and Nutrition [SR] Comments: During the inspection, Licensing Specialist observed the package of pancakes in the freezer opened and not dated. The director disposed of the pancakes putting this back into compliance. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 3.9.3, H Food Safety - H. Meal and snack menus must be planned, written, dated, and posted at the beginning of each week in an easily seen place, accessible to the parents/guardians. Any menu substitutions must be noted on the menu. A generalized menu of possible snack choices for programs that receive food donations is acceptable. All meals and snacks prepared outside of the facilitys kitchen or designated food preparation area, such as catered food, must be listed along with the source. Daily meal and snack menus must be maintained for a minimum of 12 months for licensing purposes.
