   Required background screening was missing for [ an adult or household members]. 402.313(3) F.S. and FDCH/LFCCH Handbook, Section 4.1 (Section 2.1 Health and Safety, Page 3) Staffing Requirements [SR] Comments: During review of staff and household members, there was a juvenile who now an adult without the required background screening. Juvenile screening had been expired on the birthdate of 2/18/2026. This is due to be completed by 6/3/2026. School Readiness providers must comply with the health and safety standards and requirements set by DCF and DELs health and safety requirements in this Handbook and Form DEL-SR 6200B, School Readiness Program Health and Safety Checklist. 2.1 Health and Safety DCF health and safety requirements are outlined below by provider type: F. Registered family day care homes and informal providers must comply with Rule 65C-20.008(7), F.A.C., Family Day Care Home and Large Family Child Care Home Handbook, Rule 65C-20.012(1)(h)1., F.A.C., CF-FSP Form 5318, Family Day Care Home Standards Classification Summary (May 2019) standards 1-3, 5-6, 8-32, 34-35, sub-standards 4.2-4.3, 7.5-7.10, 33.1-33.3, 33.5-33.6, 33.8-33.11, and CF-FSP 5317, Large Family Child Care Home Standards Classification Summary (October 2021) standard 4. 18.4 Background Screening All school readiness program providers and personnel are required to be screened according to the requirements of chapter 402, F.S.10 For purposes of screening in family day care homes, large family childcare homes, and informal provider homes, screening applies to any member of the home over the age of 12 years or any person over the age of 12 residing with the operator of the home. However, members of the operators family, or persons residing with the operator, who are between the ages of 12 years and 18 years are not required to be fingerprinted but must be screened for delinquency records. Initial Screening. Screening information must be documented on Form CF-FSP 5131, Background Screening and Personnel File Requirements (July 2012), which is incorporated by reference. 1. Level 2 background screening, as defined in section 435.04, F.S. is required for all personnel employed by a provider participating in the School Readiness Program pursuant to s. 1002.88(1)(e), F.S., and volunteers and substitutes as defined in this handbook, and includes a national and statewide criminal records search. 2. An employment history check must include the previous five years, which must include the applicants job title and a description of his/her regular duties, confirmation of employment dates, and level of job performance. Failed attempts to obtain the employment history must be documented in the personnel file and include date, time, and the reason the information was not obtained. 3. A copy of all background screening clearance documents for the operator, all designated substitutes, employee(s) if applicable, and all other household members who are subject to screening must be maintained in the personnel file. Re-Screening. A screening conducted under this rule is valid for five years, at which time a rescreen must be conducted in the same manner as the initial screening. 1. The five-year re-screen is required for the operator, substitute(s), employee(s), and household members between the ages of 12 and 18. 2. The five-year re-screen must include, at a minimum, national and statewide criminal records check through the Florida Department of Law Enforcement (FDLE). 3. Documentation of clearance from the five-year re-screening for the operator, all designated substitutes, employee(s) if applicable, and all other household members subject to screening must be maintained in the personnel file. 4. School readiness program personnel must be re-screened following a break in employment in the school readiness program or childcare industry that exceeds 90 days. 5. If program personnel take a leave of absence, such as maternity leave, extended sick leave, migrant childcare programs, etc., re- screening is not required unless the five-year re-screen has come due during the leave of absence. FDCH/LFCCH Handbook, Section 7.8, A, B and D (Section 2.1 Health and Safety, Page 3) Health Requirements [SR] Comments: Licensing specialist observed an infant child napping in a swing with a blanket and children under 2 sleeping in playpens with blankets and plush toys. Provider removed at time of inspection. 7.8 Appropriate, Safe and Sanitary Bedding A. A home must include a designated area where each child can sit quietly or lie down to rest or nap. Each child in care must be provided safe and sanitary bedding to be used when napping or sleeping. Linens must be washed at least once a week and more often if soiled or dirty. All bedding and linens must be thoroughly cleaned and sanitized at least once a week and before use by another child. Bedding and linens, when not in use, must be stored in a sanitary manner which prevents the spread of germs or lice from other linens. B. All personnel that care for infants must follow safe sleep practices as recommended by the American Academy of Pediatrics (AAP) as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by reference in 65C-22.001(7)(v), F.A.C. Cribs, play yards, and playpens used for infants must have tight fitting sheets and no excess bedding, which includes but is not limited to bumper pads, hanging mobiles, quilts, comforters, receiving blankets, pillows, stuffed animals and cushions. D. Children must not be placed in the cribs, playpens, play yards or other sleeping and napping bedding with items that could pose a strangulation or suffocation risk. Cribs, playpens, play yards other napping and sleeping bedding being used by a child must be placed a minimum of 18 inches away from window blinds, draperies or any window treatment/cover that pose a strangulation hazard. 17.
