Children in the outdoor play area were not adequately supervised in that [as a 2 year old child went form outside playspace back into classroom. Staff member responible was unaware child went back into classroom. ]. CCF Handbook, Section 2.4.1, A General Requirements [SR] Comments: Late afternoon on 1/23/26, mother of child was escorted to childs classroom by another staff member. Mother and this staff member discovered child playing in classroom all alone. Child was not crying or in distress. The other children and teacher were in outside play space. This teacher was unaware child was in classroom alone. Classroom tracking sheet indicated children were taken outside at 4:30pm. Parent signed child out at 4:35pm. Explanation provided by childs classroom teacher to owner was that she had 2 children who ran inside from outside play space. She stated she went to classroom door and told children to come back outside. She then closed the door without checking to see if anyone was still inside. This staff member was terminated from position on 1/28/26. 2.4 Supervision page 12 2.4.1 General Supervision Requirements B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. During the facilitys licensure year, fire drills utilizing the approved alarm system were not conducted monthly at various dates and times when children were in care [Augusst 2025 ]. CCF Handbook, Section 3.8.4, A Physical Environment [SR] Comments: The last fire drill was conducted 7/17/25. No fire drill or any other drill was completed in August 2025. A fire drill has been conducted today to put standard in compliance. 3.8 Fire Safety and Emergency Preparedness page 27 To ensure the safety of children in care, facilities are required to receive yearly fire inspections by a certified fire inspector, and they must perform monthly drills/exercises to practice fire, inclement weather and lockdown procedures. Frequent practice of emergency procedures will facilitate a calm and competent reaction in response to an actual emergency, should it occur. 3.8.4 Fire Drills A. Child(ren) did not have a Florida Certification of Immunization (DH Form 680) or a Religious Exemption from Immunization (DH Form 681), on file within 30 days of enrollment. CCF Handbook, Section 7.1, B Record Keeping [SR] Comments: One child does not have proof of immunizations as required within 30 days of enrollment. Child start date at facility was 4/26/25. Director informed parent today that child cannot attend until immunization form is provided. 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. A. Immunizations received out-of-state are acceptable; however, immunizations must be documented on the Florida Certification of Immunization form and must be signed by a physician practicing in the State of Florida. B. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations must provide Compliance Comments 67 children enrolled. 4 newly enrolled children since last visit on 5/6/25.
