Documentation of Level 2 Clearinghouse screening clearance was missing for child care personnel. CCF Handbook, Section 7.4.1,C (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Comments: Specialist observed that three newly hired staff members did not have screenings on file. Specialist verified that two of the staff members were eligible and that the third needed a resubmission due to a 90 day break in service. Provider stated that she had issues accessing the system. Specialist provided technical assistance. H. The employer/owner/operator must maintain on-site at the program copies/documentation of completion of all applicable elements in the screening process for an individual in the personnel file for review by the licensing authority. The facility did not have electrical outlet covers or the outlet covers were not in place. CCF Handbook, Section 3.1, I (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: All accessible electrical outlets must be tamper-resistant electrical outlets" that contain internal shutter mechanisms to prevent children from sticking objects into receptacles. In settings that do not have "tamper-resistant electrical outlets", outlets shall have safety covers. There was a broken electrical outlet that did not have a cover. It is located in the front area of the daycare where the after-school program and four year old students are located. 13.
