The play equipment was not maintained in a safe or sanitary condition for the children to use in that [The paint is chipping on a couple of pieces of equipment and the swings chain covers are in bad condition.]. CCF Handbook, Section 3.12, B and F Sanitation and Equipment [SR] Comments: Specialist observed on the playground that a play car with steering wheels, the paint is chipping, the slide stair railing is also chipping and the swings chain covers are in bad condition are hanging half off. Provider has until April 30, 2026, to repaint and fix the chain covers on the swings. The Child Care Facility Handbook states on page 38-39: B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. F. All equipment used in the outdoor play area must be constructed and maintained according to manufacturers recommendations and allow for water drainage. CCF Handbook Section 4.2.1 Comments: Specialist reviewed all staff records. One staff has not completed their training in the allotted time. The Child Care Facility handbook states on page 40: 4.2.1 Mandated Introductory Training Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Child care personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004 are not required to fulfill the competency examination requirement. DH Form 680 and DH Form 681 may be obtained from the local county health department.
