The operator, substitute and/or volunteer did not have a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. FDCH/LFCCH Handbook, Section 4.3, H (Section 2.1 Health and Safety, Page 3) Staffing Requirements [SR] Comments: During the inspection, the operator advised that there is currently 1 substitute, and 2 household members. During the inspection, the Licensing Specialist reviewed 4 files. During the inspection, the Licensing Specialist observed that there were two CF-FSP 5337 Child Abuse and Neglect Reporting Requirements forms that were not signed annually. Photos of the non-compliance was taken during the inspection. The Operator has been given a due date of July 27, 2025, to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 4.3, H: Background Screening Documents: H: The operator, substitute, employee and/or volunteer must complete a CF-FSP 5337, Child Abuse and Neglect Reporting Requirements, incorporated by reference in 65C-22.001(7)(l), F.A.C., signed prior to initial licensure and annually thereafter. The outdoor play area that required fencing was not safe and adequate in that [the fence had an opening leading to the backyard area]. FDCH/LFCCH Handbook, Section 7.6, F (Section 2.1 Health and Safety, Page 3) Health Requirements [SR] Comments: During the inspection, the Licensing Specialist observed that a portion of the fence surrounding the play area led to the backyard area that had a shed and a lid to a bin that was not in good repair. The Operator advised the Licensing Specialists that the fence was damaged due a storm. The Operator has been given a due date of 7/7/2025 to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 7.6, F: Outdoor Time, Fencing and Play Area Requirements: F: Fencing, including gates, must be continuous, and shall not have opening or gaps larger than 3 1/2 inches that would allow children to exit the outdoor play area. The bottom or base of the fence must remain at ground level and free from erosion or buildup to prevent inside or outside access by children or animals.
