Operator failed to maintain a current Employee/Contractor Roster for substitutes, employees or household members in the Clearinghouse. FDCH/LFCCH Handbook, Section 4.1, L Comments: During the inspection, the Licensing Specialist observed the operator failed to maintain a current Employee/Contractor Roster for substitutes, employees or household members in the Clearinghouse. This standard has been brought back into compliance in that the operator updated the clearinghouse during the inspection. During the inspection, the Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 4.1, L: Initial Screening: Operators, household members (adults and children 12 to 17 years of age), substitutes, volunteers and Large Family Child Care Home employees must have a level 2 background screening clearance from the Department prior to obtaining a license, residing in the home, employment, or volunteering unsupervised with children. The employer/owner/operator must review each employment application to assess the relevancy of any issue uncovered by the complete background screening, including any arrest, pending criminal charge, or conviction, and must use this information in employment decisions in accordance with state laws. L. The employer/owner/operator must add substitutes, employees and household members to their Employee/Contractor Roster within ten days of when the individual has received a child care eligible result. Employer/owner/operator must add an end date within ten days of termination for individuals on the Employee/Contractor Roster in the Clearinghouse when employment terminates, or a household member is no longer residing in the home. FDCH/LFCCH Handbook, Section 5.1.2, A or B Comments: During the inspection, the Licensing Specialist observed the substitute, who worked less than 40 hours a month on average, did not complete the departments 6 clock-hour Family Child Care Rules and Regulation course (or have documentation of completion of the 3 clockhour Fundamentals of Child Care Home Rules course.) as evidenced by the Departments training transcript. The Operator has been given until 7/4/2026 to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 5.1.2, A or B: A. The Departments six clock-hour Family Child Care Home Rules and Regulations training, as evidenced by successful completion of a competency-based examination offered by the Department or its designated training representative prior to licensure and caring for children. Substitutes who have successfully completed the three clock-hour Fundamentals of Child Care training or 30-clock-hour Family Child Care Home training are not required to complete the six clock-hour Family Child Care Home Rules and Regulations course. Medication which had expired or was no longer being administered was not returned to the custodial parent or legal guardian or discarded if the child is no longer enrolled. FDCH/LFCCH Handbook, Section 7.24, I Comments: During the inspection, the Licensing Specialist observed non-prescription medication diaper cream which had expired. The diaper cream is Aquaphor: Expiration Date: 5/2024. This standard has been brought back into compliance in that the operator discarded the diaper cream into a trash receptacle that is inaccessible to children. During the inspection, the Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 7.24, I: Medication: The family day care home or large family child care home is not required to give medication; however, if they choose to do so, the following shall apply: I. Medication which has expired or is no longer being administered shall be returned to the custodial parent or legal guardian or discarded if the child is no longer enrolled in care at the home. The Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)].FDCH/LFCCH Handbook, Section 8.1 A and B Comments: During the inspection, the Licensing Specialist observed 1 Florida Certificate of immunization was not acceptable in that the form was not current (expired). This standard has been brought back into compliance in that the child was taken home by the child's custodial parent/guardian during the inspection. The Operator informed the Licensing Specialist that the immunization record would be updated before the child returns to the home day care. During the inspection, the Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 8.1, A: Health/Immunization Records A. The operator is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form, Parts A-1, B, and/or C, DH 680 (July 2010), or the Religious Exemption from Immunization form, DH 681 (July 2008), which are incorporated herein by reference in 65C-22.001(7)(o) and (p), F.A.C., from the custodial parent or legal guardian, within 30 days of enrollment. DH Form 680 and DH Form 681 may be obtained from the local health department. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations should provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations.
