The operator, substitute and/or volunteer did not have a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. FDCH/LFCCH Handbook, Section 4.3, H Comments: At the time of inspection, the Licensing specialist observed the child care personnel listed on the supplemental did not have the Child Abuse and Neglect form signed annually. Staff 1 last signed the form on 4/8/2025. Staff 2 last signed on 4/10/2025. The operator immediately had the staff sign the form at time of inspection. TA was provided to ensure that the Child Abuse and Neglect form is signed annually by the due date. Compliance Comments Specialist reviewed the operator, substitute and household member files. The home does not have documentation for all child care personnel on the required annual refresher education of the homes exposure plan. FDCH/LFCCH Handbook, Section 7.23 Comments: At the time of inspection, the Licensing specialist observed the child care personnel listed on the supplemental did not complete exposure plan training annually. Staff 1 and 2 both completed the training last on 4/9/2025. TA was provided to ensure that the exposure plan training is completed by the due date and annually every year there after. Not Applicable Comments At the time of the inspection, the provider stated they do not dispense medication to children, including diaper rash cream, sunscreen, insect repellant or any life- sustaining medications such as inhalers, epi pens, etc. No medication was observed.
