FDCH/LFCCH Handbook, Section 8.2, A Comments: On todays date the Licensing specialist called, texted and emailed the provider to inquire if the child listed on the supplemental has a current medical physical on file. The provider advised that the mother of the child was told by the doctors office that they do not complete physicals until the child is at least age one. The child is an infant and will be one soon. The specialist advised the provider that the child is able to have the required form, DH 3040 filled in by a physician for the child. The child has been enrolled in the home since 11/17/2025 and is still currently enrolled and attending. TA was provided to ensure that the child has a current medical physical on file by the due date. 33. FDCH/LFCCH Handbook, Section 7.14, A Comments: At the time of inspection, the Licensing specialist determined the provider did not have a written/ planned menu posted for the childrens meals. The provider advised that she has not had a menu and no longer has a contract with the food program. The specialist reminded the provider that a written weekly menu is required to be posted that includes all required food groups. The provider mentioned she verbally tells parents daily what food is offered. At time of inspection, the provider wrote out a menu for the rest of the current week. TA was provided to ensure that a weekly menu is posted at the beginning of each week in an inconspicuous area where parents can view. FDCH/LFCCH Handbook, Section 7.21, D and 7.22, C Comments: At the time of inspection, the Licensing specialist and operator conducted a fire drill. The operator failed to possess attendance and emergency contacts for all children present. The specialist advised the provider that we will do the fire drill again. Another drill was conducted and the operator possessed all required information. The specialist encouraged the provider to possess attendance and contacts for emergencies for each monthly drill and also during an actual emergency. TA was provided to ensure that the operator possess both attendance and emergency contacts during drills and an actual emergency. At the time of the inspection, Licensing counselor observed the providers last fire drill was conducted on___4/3/2026________. The provider conducted a lockdown drill on____________and a weather drill conducted on__4/12/2026_______. The fire extinguishers were last tagged on August 2025. TS was provided to ensure that the operator conducts a lock down emergency drill by the end of the current licensing year by, 4/30/2026.
