3.9.3(G), Handbook: If a special diet is required for a child by a physician, a copy of the physicians order, a copy of the diet, and a sample meal plan for the special diet must be maintained in the childs file and followed. If the custodial parent or legal guardian notifies the program of any known food allergies, written documentation must be maintained in the childs file for as long as the child is in care. Special food restrictions must be shared with staff posted in an easily seen location and followed. Food was served to a child with an allergy or dietary restriction. Food Service/Nutrition [SR] Comments: On 01/23/2025, CCLE received a complaint in which alleges a parent provided dietary restriction for no meat for her child and the Childcare gave meat to the child. Findings: A complaint inspection was conducted on 01/27/2026 and a statement was received from the director that the child was provided a lunch form home. When lunch was served, the cook brings the plated food into the classroom and places them on the tables. The child sat down in front of her lunch and the school lunch being served. Per the director, the beef portion of the lunch is typically removed before the child can access it. In this case, the child ate the beef before the classroom teachers could remove it from her plate. The childs file was reviewed and there was a physicians note explaining the dietary restrictions: No Beef or Pork. The childs allergy/food restriction was posted in the proper places. The Director was unaware of this incident until the parent complained that the child was having a reaction. This complaint is substantiated. This was put back into compliance by the provider changing the food serving procedures to allow for active supervision while food is being served. CHILDRENS RECORDS 36. 7-33(d), Ordinance: When children are napping, the child care direct supervision requirements set forth below shall apply in addition to the staffing ratio requirements: (1) If any of the napping children are under 12 months of age, there must be one child care staff member for every four children. (2) If any of the napping children are under 2 years but over 12 months of age, there must be one child care staff member for every six children. (3) If the napping children are all at least two years of age but none are three years of age or older, there must be one child care staff member for every 22 children. (4) If the napping children are all at least three years of age, there must be one child care staff member for every 30 children. A staff-to-child ratio of [2] staff for [18] children was observed. Comments: The following ratios were observed; Infants - 1:4, 1s and 2s 2:18, 3s and 4s 1:12. The mixed group of 1s and 2s was found to be out of ratio.
