3.5(A), Handbook: The outdoor play area must be clean and free from litter, nails, glass and other hazards. The following hazard was observed in the outdoor play space [When transitioning from concrete to mulch it was found that the concrete had developed cracks and broken pieces along the edges at two corners. ]. (Section 2.1(A) Health and Safety, Page 3) Indoor/Outdoor Space [SR] Comments: When coming out the door leading to the playground and walking on the concrete, a section can be seen with multiple concrete cracks. This is very apparent were the concrete meets the mulch and broken pieces along the edges were present at two corners. This is due to the cement having no support. This inspection was signed by the provider on 2/3/26 and revised on 2/5/26. 22. 3.8.2 (A), Handbook: Unless statutorily exempted, all child care facilities must conform to state standards adopted by the State Fire Marshal, Chapter 69A-36, F.A.C., Uniform Standards for Life Safety and Fire Prevention in Child Care Facilities. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority. The facility has not been inspected and approved by the local fire department as required. (Section 2.1(A) Health and Safety, Page 3) License/Certificates [SR] Comments: The facility had a fire inspection that expired 1/16/26. The facility had notified the fire dept and was on the schedule for their inspection in the coming week.
