3.1(A), Handbook: All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. An area(s) of the facility was observed to not be in good repair. Facility/Storage [SR] Comments: The following areas of the facility were found to be in need of repairs, - The red and blue torn naptime mats, crib mattresses, and diaper changing pads - Add classroom materials to the centers and organize the classroom materials - Additional mulch playground area (underneath slides and swings) - Remove broken infants toys from underneath the shaded area - Secure tarp to the shaded structure - Repair the red and blue swing playground structure (displaying rust) - Post evacuation routes, inclement weather schedules, emergency phone numbers, directions, handwashing signs and capacities - Clear out the shower ( can not be used as a storage) - Light fixtures operable condition - Outlet covers - Rodent traps need to be placed where children do not have access to them - Torn wallpaper (infant/toddler classroom) - Remove broken chairs - Source: FL DCF CARES inspection 2025-01-27. Sec. 7-32, Ordinance: Before the first day of employment at a child care facility, all child care personnel, excluding temporary substitutes, must have on file at the child care facility a signed statement from a licensed physician or authorized agent of the Florida Department of Health attesting that the person is in good health in order to care for children in a child care facility setting. The health assessment statement shall be documented on a form prescribed by the local licensing agency and updated at least every two years. Child Care personnel did not have a current health statement on file. Comments: There were four (4) staff member files observed without current No. 8 forms. 06. 3.1(A), Handbook: All child care facilities must be in good repair, free from health and safety hazards. Areas of the facility were found to be in need of repair in the following areas [See below]. Facility/Storage [SR] Comments: The following areas are in need of repair: Peeling paint in the 1-year-old classroom. • Bathroom light switch plate needs to be installed • Light cover in school age room needs to be installed • Protruding staples on the walls need to be removed. • One swing was observed to be broken, and another was inaccessible. • Compliance Comments Technical assistance was provided regarding keeping cleaning supplies, medicine and staff handbags in a locked cabinet/drawer. Sec. 7-5.01(f), Ordinance: Medicines, cleaning supplies, flammables, and other potentially poisonous supplies shall be stored and locked in a place out of a child's reach and in such a manner as to ensure the safety of the children. All surfaces or items accessible to children shall be free of toxic materials, and pose no threat to the health and safety of the children or personnel. Sec. 7-6.01(c), Ordinance:The child care facility is required to have at least two (2) adult staff members who have first aid and Pediatric Cardiopulmonary Resuscitation (CPR) training and at least one (1) of these staff members shall be on the premises at all times that children are present in the facility.The required amount of trained staff were not on the premises. 7.5 and (A), Handbook:Daily attendance of children must be taken and recorded accurately by the child care personnel, documenting the time when each child enters and departs the program.Attendance devices used for the purposes of tracking attendance may be used, but personnel must ensure the accuracy of the documented attendance. Each classroom must have an attendance sheet/class roster for the group of children occupying that space.There was no attendance sheet/class roster in the [2 year old ] classroom.
