Sec. 7-32, Ordinance: Before the first day of employment at a child care facility, all child care personnel, excluding temporary substitutes, must have on file at the child care facility a signed statement from a licensed physician or authorized agent of the Florida Department of Health attesting that the person is in good health in order to care for children in a child care facility setting. The health assessment statement shall be documented on a form prescribed by the local licensing agency and updated at least every two years. Child Care personnel did not have a current health statement on file. Comments: A staff member did not have a current medical statement on file. 06. 2.8 (B), Handbook: The child care facility operators, employees, and volunteers must comply with written disciplinary and expulsion policies. Child care personnel did not comply with the facility's written disciplinary policy. Ratios/Discipline [SR] Comments: The director of the facility contacted Childcare licensing on 1/15/2025 to report and incident that occurred at her school on 1/14/2025.The director stated that she observed through a video one of the staff members roughly handling a child and placing her on her cot during nap time. The director stated that as result of this action the child had a small bump on the forehead. The director terminated the child's teacher and scheduled a meeting with the child's parents to discuss the incident. FACILITY/STORAGE 10. 5.1(K), Handbook: The employer/owner/operator must add child care personnel to their Employee/Contractor Roster in the Clearinghouse when the individual has received a child care eligible result and has been hired at the facility. Employer/owner/operator must add an end date for individuals on the Employee/Contractor Roster in the Clearinghouse within 10 days of the employment termination.The Clearinghouse roster was not updated. Staff Requirement [SR] Comments: The clearinghouse roster was not updated for a staff member who serves as a therapist for one child during the day. The staff member has an eligible ACHA screening for her role as a therapist during the day but requires agency review for DCF childcare eligibility for role as childcare staff in the afternoon.
