Sec. 7-32, Ordinance: Before the first day of employment at a child care facility, all child care personnel, excluding temporary substitutes, must have on file at the child care facility a signed statement from a licensed physician or authorized agent of the Florida Department of Health attesting that the person is in good health in order to care for children in a child care facility setting. The health assessment statement shall be documented on a form prescribed by the local licensing agency and updated at least every two years. The licensing monitor brought this concern to the attention of the teacher and school director. The director immediately removed the cleaning solution, bringing this standard into compliance. Indoor/Outdoor Space [SR] Comments: At the time of inspection, the monitor observed open containers (mop buckets) of water were not emptied after use in multiple classrooms. The licensing monitor brought this concern to the attention of the teachers and the school director. The teachers immediately discarded the water bringing this standard back into compliance. 3.9.6(G), Handbook: Due to the extreme risk of choking, solid foods, including cereal, may not be given in bottles or with infant feeders to children with normal feeding habits unless authorized by a physician. Solid foods may not be fed to an infant younger than 4 months of age unless directed by a physician. Solid foods must be of a safe consistency and must be developmentally appropriate for the age and developmental ability of the infant. Solid food and/or cereal was fed to an infant under 4 months of age in a bottle and/or without authorization from a physician. 2.5.2(F-G), Handbook: Upon arrival at the destination by vehicle, the driver of the vehicle must mark each child off the log as the child departs the vehicle; conduct a physical inspection and visual sweep of the vehicle interior to ensure that no child is left in the vehicle; and record, sign, and date the transportation log immediately, verifying that all children were accounted for, and that the visual sweep was conducted; ensure that a second child care personnel conducts a second visual sweep, signs and dates the transportation log verifying that all children were accounted for, and that the log is complete. If the provider contracts with an outside entity to provide transportation, then the provider must assign a child care personnel to perform the duties of the driver. Upon arrival at the destination by vehicle, a second and different child care personnel must conduct a physical inspection and visual sweep of the vehicle to ensure that no child is left in the vehicle; and sign, date and record the transportation log immediately, verifying that all children were accounted for, and that the log is complete. Procedures were not followed in that [see details below]. Transportation [SR] Comments: The licensing monitor observed destination procedures were not followed in that upon arriving to the program the driver did not immediately mark off each child as they departed from the vehicle to verify all children were accounted for. While inspecting the additional vehicle the monitor had to prompt the school director to complete the second sweep as that vehicle was leaving. Technical Assistance: F. Upon arrival at the destination by vehicle, the driver of the vehicle must: 1. Mark each child off the log as the child departs the vehicle; 2. Conduct a physical inspection and visual sweep of the vehicle interior to ensure that no child is left in the vehicle; and 3. Sec. 402.305(4)(a), Florida Statutes: Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. A ratio of [2] child care personnel for [7] children is required. A ratio of [1] child care personnel for [7] children was observed. Ratios/Discipline [SR] Comments: At the time inspection, the monitor observed office manager leave the front office area and removed a one-year old child from the 1's classroom into the infant classroom. Per the office manager, the child's parent just arrived and placed the child into the 1's classroom. The monitor verified the time the child was signed in on the classroom roster was 30mins prior to inspection. The monitor informed staff on site that the classrooms need to remain in ratio when the children arrive. Provider resolved standard by correcting room ratio and roster entry. Sec. 7-43(a), Ordinance: Within 15 days after enrollment, the child care facility must obtain, from a parent for each child in care, a current, complete, and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in Rule 65C-22.001(7)(o), Florida Administrative Code, or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in Rule 65C-22.001(7)(p), Florida Administrative Code. DH Form 680 and DH Form 681 may be obtained from the Florida Department of Health. The Florida Certificate of immunization was missing for children in care.
