6.4(A), (B) and (D), Handbook: All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form. Documentation of an accident/incident was not provided on the day it occurred. Childrens Records [SR] Comments: Complaint alleges that the child (J.) had a hematoma on his forehead when he was picked up from school. The injury was not reported. J. also has a left brachial plexus injury. FINDINGS: A complaint inspection was conducted on 6/23/2026 and a statement from a child care personnel was received. Per the child care personnel (C.F.), she is the VPK teacher in the mornings and the office staff person in the afternoon. On the date of the alleged incident, she was serving lunch and recalls J.M. falling. She stated that he is learning to walk and due to the injury that he has on his left side, he struggles to maintain balance. He walked between two children that were sitting at the toddler table, and fell forward, hitting his forehead on the table. He did not cry for long and she did not observe a knot on his forehead. When he fell, she saw a slight little knock on the table, meaning that the hit was not hard. Per the director, The child was picked up by the aunt on 6/8/2026 and aunt did not say anything about a mark on the child. After further investigation, they observed the child falling on the floor with the aunt after she pickup him up. He may have received the injury to the forehead from hitting his forehead on the concrete floor. Video of both falls were observed. Child seems to fall while he was standing and hit his head on the toddler table as he was falling forward. The fall does not appear to be hard, as he fell slowly from standing down to his knees and forward toward the table. Once the family member picked up, the family member is seen putting him down to stand and the child fell on his bottom. She held him by one hand, until he gained balance, then he took a couple steps, lost his balance, fell to the floor, and appears to hit his face on the concrete floor. This complaint is partly substantiated for not providing a copy of the accident/incident form to the person picking up the child (aunt). 41. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Asphalt, concrete, hard packed dirt, hay, grass or leaves are unsuitable for use in the use zone area. If the ground cover in place is a unitary playground surface, then the unitary surfacing materials must be installed, maintained, or replaced according to manufacturers instructions. Sec. 7-33(e), Ordinance: Direct supervision means watching and directing the childrens activities within close proximity within the same room inside or within a designated outdoor play area and responding to each childs needs. Inadequate supervision occurred when a child/childrens needs were not met in the following incident: [Child received scratches and bruise on face]. Ratios/Discipline [SR] Comments: The school director self reported that An incident occurred today at Greater Horizons Academy involving a child at the gym during playtime. The teacher was attending to another child who was acting up, and when she turned back around, she saw that the little girl was on the floor. The teacher went to check on her and noticed a scratch on her face and a bruise on her cheek. The teacher said she didnt know who did it because she was attending to another student. We called the parent and filed an incident report. The parent was upset and wanted to take further action, so I reported it on the DCF website. FINDINGS: A complaint inspection was conducted on 9/30/2025 and a statement from the teacher was received. Per the teacher, the incident occurred on Friday afternoon. The 2y/o were at playtime in the gym. She has an autistic child that is very active. When they go to the gym and they play with a ball they do not like to share. So he was acting up really bad, throwing a tantrum. When she turned around and looked as SM, the child was on the floor. When she picked her up she saw that one of the students scratched her up in her face. The teacher did not see what happened or who scratched the child. She just saw the marks on the child when she picked her up off the floor. The teacher was asked if there is a way for her to communicate to office staff when she needs help with challenging behaviors and she stated that she has to leave the room with the whole group. I provided TA to the director about using an efficient means of communication, like a walkie talkie to provide support to the staff. A non-compliance was issued for inadequate supervision as the teacher was not watching and directing the children's activities when the incident happened. 08.
