If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Asphalt, concrete, hard packed dirt, hay, grass or leaves are unsuitable for use in the use zone area. If the ground cover in place is a unitary playground surface, then the unitary surfacing materials must be installed, maintained, or replaced according to manufacturers instructions.    2.4.4(B), Handbook: Children must not be left in high chairs or other types of feeding chairs other than feeding times. A child was observed in a high chair outside of feeding times. Food Service/Nutrition [SR] Comments: During the course of a complaint inspection, the following information was received; The director stated and showed me a photo of the 1y/o child in question, painting/coloring while sitting on feeding chair. TA was provided that children should not be placed in feeding chairs during non-feeding times. The child's main teacher (A), stated that when they went out into the playground the child did not get injured. The reason that the child did not get injured is because she was sitting on an a baby chair or in her arms during the duration of the playground time. At around 4:30PM, the child was changed and sat on an infant bouncy seat. The main teacher left at around 4:30PM and another teacher stepped in to cover the class. The second teacher that covered the class corroborated the main teacher's statement that the child was sitting in a bouncy seat in the afternoon. The second teacher picked her up off the bouncy seat to check her because she was fussy on and off, and seemed uncomfortable. Medication Requirements Group: DCF Handbook Not Monitored Compliance Comments Complaint alleges that on Friday, 5/2/25 child had scratches on her child's left shoulder. FINDINGS: A complaint inspection was conducted on 5/12/2025 and a statement from the director was received. Per the director, she was not on-site when the incident happened. She received a call notifying her that a teacher noticed marks on the child. They contacted the parent to notify the, of the marks. An incident report was written and issued to the parents at pickup time. The parents refused to sign the incident report. Per the child's teacher, she didn't see any marks on the child. She stated that the child's shirt covered her back so they wouldn't have been able to see it if the child arrived that way. The child had a normal day and did not cry throughout the day. She changed her clothes during diapering time at the end of the day and didn't see the marks on the child. The teacher left for the day, leaving the child sitting on a baby rocker/ seat. Another teacher went in to replace the main teacher. Per teacher B, she noticed that the child was fussy and kind of moving her back side to side on the seat while she sat on the baby rocker. The teacher thought the child may have had a bug on her back or something else bothering her so she picked her up and checked her clothes. At that time, she noticed the marks on the child's back and reported it to the office staff. An accident/incident report was created and given to the parents.
