6.4(A), (B) and (D), Handbook: All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form. Documentation of an accident/incident was not provided on the day it occurred. Childrens Records [SR] Comments: A complaint inspection was conducted on 10/8/2025. The Kinderworld school bus left a child at the elementary school. Therefore, a sister school bus (Kindergarten Infant Daycare Specialists) picked up the child and transported her to their school. Per the director for Kindergarten Infant Daycare Specialists, the child was going to be transported to the right school at a later time.Based on the statement received from Kinderworld school director IA, an incident report was not documented since this happens on multiple occasions. Per IA, the program was short staffed and called Kindergarten Infant Daycare Specialists to pick up the child. This complaint is unsubstantiated, but due to the lack of documentation of unusual occurrence while the child is in care was not provided on the day it occurred this standard is out of compliance.This concern was brought to the attention of the director and program board member. The director emailed the parent communication regarding the unusual occurrence on 10.08.25, bringing this standard into compliance. 41. (2) Operate or attempt to operate a child care facility or family child care home without having obtained a license required by this chapter. (3) Operate or attempt to operate a child care facility or family child care home under a license that is suspended, revoked, or terminated. (4) Misrepresent, by action or omission, a child care facility or family child care home to be duly licensed pursuant to this chapter without being so licensed. (5) Make any other misrepresentations, by action or omission, regarding the licensure or operation of a child care facility or family day care home to a parent who has a child placed in the facility or home or who is inquiring as to placing a child in the facility or home, or to a representative of the local licensing agency, or to a representative of a law enforcement agency, including, but not limited to, any misrepresentation as to: a. The number of children at the child care facility or the family child care home; b. The part of the child care facility or family child care home designated for the provision of child care; c. The qualifications or credentials of child care personnel at the child care facility or home; d. Whether a child care facility or family child care home complies with the screening requirements of Section 402.305, Florida Statutes; or e. Whether the child care personnel have the training as required by Section 402.305, Florida Statutes. Child care personnel misrepresented information, impersonated, or provided fraudulent information related to the child care facility to a parent/guardian, licensing authority, or law enforcement. License/Certificates [SR] Comments: A complaint alleges that a woman named Nadine has been at this facility over 6 months working primarily with the after-school program. Nadine is unlicensed and she has not attended the necessary hours of training and testing to qualify to work at this facility. Nadine often is sheltered during inspections, shuffling her to different rooms to avoid being seen or Nadine leaves the facility and returns thru the front door acting like a parent when an inspection is in-progress. Nadine is there daily with after schoolers. A complaint inspection was conducted on 12/11/2024 and statements from all the teachers and director(I. Ashbourne) was received. A walkthrough of the facility was done and the following was observed; Upon arrival, the director was informed that I was there to conduct a complaint inspection and needed to do a walkthrough of the school. She asked what the complaint was and she was informed that we would discuss the allegations after my walkthrough. We proceeded to the first classroom on the left side of the hallway. At this time, i said I want to see aftercare first. Then, we proceeded to the after care room through the hallway back door. The hallway door leads into the playground and the aftercare room sits adjacent to the playground. In the playground, I saw a teacher (Ms. Waennika) watching some of the after schoolers and when I glanced into the aftercare room I saw two adults and some after schoolers in the room. As we walked toward the aftercare room and Ms. Waennika did too and stood by the door talking to Ms. Zamin, I looked into the room, and saw Ms. Zamin and another individual. This individual was a black woman, she wore a black shirt, had a low haircut and her hair was blonde at the top and black at the sides. The unknown individual rushed through a door in the room the led into the kitchen. I couldn't walk into the room because Ms. Waennika was standing by the doorway. I said excuse me so she moved and I was able to get by. At that time, I rushed into the kitchen looking for the woman in question. I proceeded into all the other classrooms and the woman was nowhere to be found inside the building. My co-worker (A. Joyce) that was outside the building saw the woman run out the front door, she got into a black car and drove to the store that was adjacent to the school. Statements from the staff: The director was asked who the individual was that was in the aftercare room to which she responded that she doesn't know and that it may have been a parent. In my observations that day and past visits, all parents that have been seen picking up children, pick up in the office and are not allowed in the hallways and classrooms. Some staff stated that they didn't know who I was talking about and denied seeing the individual in the building. When asked directly if they know a Nadine and if she was in the building, a teacher confirmed that she was the woman i described. The aftercare teachers that would have seen the person in question in that room with them, denied seeing her or knowing the individual in question. They were informed that they were interfering with my investigation because the woman was working with them and even handed an object to one of them before storming out. FINDINGS: Quality Assurance Specialist (Q. Gomez) saw an individual at the school that the staff is failing to or unwilling to identify. School director says it could be a parent but failed to identify the parent. The staff claim to not have seen the individual or not know of anything that happens out of their class. Child Care Licensing Monitor (A. Joyce) saw the individual storming out of the school and into her car before taking off. A non-compliance is being issued for providing misrepresented information to Child Care Licensing and Enforcement. 46. The licensing monitor brought this concern to the Director and teachers on site. Sec. 7-44(f), Ordinance: The childs arrival and departure must be recorded by child care personnel at the time the child enters and departs the child care facility. The attendance sheet/class roster must accompany the child care personnel and the group of children throughout the day should they leave the classroom. In order to account for childrens whereabouts, children who are reassigned to different classrooms throughout the day should be signed in and/or out of the rosters for those respective classrooms each time. The facility failed to maintain an attendance roster for the children in care.
