The areas to include the flooring at the door way of the infants, two and VPK classrooms were observed with the lamenant material shredding exposing wood pieces or missing and peeling. Also, in the infant classroom near the light switch paint was missing. In the outdoor of the facility the lattus fence material was added to the fencing on the playground was not maintained and was observed broken with missing pieces. There were wires exposed on the outside of the building where a light fixture once was. In the VPK classroom near the left hand side above the television there was a hole in the wall with a blue cord inside most likely for the camera. Lastly, in all of the restrooms throughout the facility the handicap grab bar assist poles were observed with rust like stains.
