Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. 03. Ratio Sufficient CCF Handbook, Section 2.3 (03-01)

A ratio of [1] child care personnel for [6 one year old ] children is required. A ratio of [1] child care personnel for [29 ages 1 year to 5 years old] children was observed. s.402.305(4), F.S. General Requirements [SR] Comments: Specialist observed ratios out of compliance. From 7:43 am to 8:04 am, there was only 1 qualified staff member supervising 29 children ages 1 year old to 5 years old. Children were calm and engaged with watching cartoons on television. Ratios in compliance at 8:09am with arrival of more staff. Specialist gave technical assistance to Director to ensure staff schedules will accommodate required ratios. 2 General Requirements page 11 and 12 2.3.1 Mixed Age Groups A. In groups of mixed age ranges, where children under one year of age are included, one child care personnel must be responsible for no more than four children of any age group, at all times. B. In groups of mixed age ranges, where children one year of age but under two years of age are included, one child care personnel must be responsible for no more than six children of any age group, at all times. A ratio of [1] child care personnel for [6] children is required. A ratio of [1] child care personnel for [11] children was observed. s.402.305(4), F.S. General Requirements [SR] Comments: Observed during naptime 11 one-year olds with 1 staff member. All children were sleeping. Another staff member was added when Director was informed of situation. This was corrected within 5 minutes on 10/21/24 2.4 Supervision Page 13 and 14 2.4.2 Nap time Supervision During nap time, child care personnel must be within sight and hearing of all the children. All other child care personnel required to meet the staff-to-child ratio must be within the building on the same floor and available to be summoned if needed to ensure the safety of the children. 33. Training Requirements CCF Handbook, Section 4 (33-01)

Child care personnel including volunteers who work 10 hours or more per month did not complete the 40 hour Introductory Child Care Training requirement. CCF Handbook Section 4.2.1 Comments: One staff member did not compete required 40 training which was started on 7/31/23. 4 Training page 40 and 41 Training requirements of this section do not apply to child care personnel who do not work directly with the children such as cooks, book keepers, and janitors who are considered other personnel. If at any time other personnel fill in for classroom child care personnel and are in direct care of the children, training is required. Child care personnel in compliance with the school-age requirements shall be considered in compliance with the child care personnel training requirements. 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online child care training course, or by receiving results from a Department-approved competency examination. The child care program is responsible for obtaining training documentation from child care personnel. 4.2 Training Requirements 4.2.1 Mandated Introductory Training Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Child care personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004 are not required to fulfill the competency examination requirement. A. The facility did not have documented proof that all child care personnel were trained and knowledgeable within 30 days of date of hire in [ safe sleep practices and shaken baby syndrome]. CCF Handbook, Section 4.2.3 and 4.2.5 Training [SR] Comments: Two staff members have no proof of completing Safe Sleep / Shaken Baby Syndrome Training as required within 30 days of hire. This was verified on transcripts dated 10/21/24. 4.2 Training Requirements pages 40 and 41 4.2.3 Safe Sleep/ Shaken Baby Syndrome Training All child care personnel who work in a facility that offers care to infants must have training regarding guidance on safe sleep practices, preventing shaken baby syndrome and abusive head trauma; recognition of signs and symptoms of shaken baby syndrome and abusive head trauma; strategies for coping with crying, fussing, or distraught child and the development and vulnerabilities of the brain in infancy in early childhood within 30 days of hire at the facility. For child care personnel to satisfy this requirement the training must be accomplished through one of following methods: Department's Health Safety and Nutrition course, Safe Sleep course, or the Early Learning Floridas Safe Sleep Practices.
