LC Truss received an incident report to the e-licensing portal on April 15, 2026. That a child was left unattened. The classroom was transitioning to another classroom around 4:45pm and the child got out of line after name-to-face was called and went to the restroom. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: We went around to every staff member and had them sign our playground/supervision policy. We also have them calling the admin to help with any transitions throughout the day etc., playground, and transitioning to other classrooms in the afternoons. We wrote up/termed the educator in question. We are getting into contact with CCR&R to come to our next staff meeting.

The infant classroom was out of ratio on 9.22.25, running at a ratio of 2:9 with infants ages 8m-15m — Visit: Complaint Investigation,Technical Assistance — Action taken: We discussed proper ratios with management and staff members. The ratio chart was re-given to all staff members for further clarification. If there's ever any confusion about mixed age grouping, I will contact my licensor beforehand. Center management is keeping up with daily count sheets and scanning them to our regional director for a second set of eyes upon completion.
