No incident report was given to the parents or in the children's file — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: Verbally notified parents on the day of the incident on 10/17/24. Admin completed a written incident report on 10/22/24 that includes a description of incidents, staff signatures, director signatures, and parent signatures. The incident report is placed in the child's file.

LC Truss received an incident reported to the e-licensing portal on October 17, 2024. Stating that two children were left in a classroom unattended for approximately 1-2 minutes before another educator found them and returned them to their classroom room — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The educator received a write up placed in staff file. The educator cannot be left alone with children until Supervision training is completed through CCR&R. All educators and admin team have undergone Supervision training as a center through CCR&R on 10/17/24. Admin will continue to observe teachers complete Name to Face checks and counts during all classroom transitions. Documentation of Name to Face is located in our Attendance Tracking System (Classroom/Family App).
