LC arrived at Cole Academy and immediately obtained ratios in each classroom. The Creeper classroom (18m-30m) had a ratio of 1:6 and then another arrived at the classroom became out of ratio 1:7. The agency will also make sure the staff is trained in what correct ratios are for each classroom.

In the afternoon around 5:30 pm the agency combines children ages 13 months to 5 years old together — Visit: Complaint Investigation — Action taken: The agency will not combine the mixed ages in the evenings.

1240-04-01-.11 Supervision

A Child was left unattended for approximately 8.5 minutes — Visit: Complaint Investigation — Action taken: The staff members will supervise the children more closely at the end of the day when parents are picking up children.

1240-04-01-.06 Staff

Management is responsible for hiring qualified staff, and then supervising their performance to ensure they are capable of their job responsibilities. The management's failure to do this has led to a staff member using inappropriate language and behavior with the children. — Visit: Complaint Investigation — Action taken: The staff member has been terminated from his position at the school . The management will screen potential applicants carefully to ensure qualified staff are hired. When an applicant is hired, the management will supervise the staff closely to ensure they are capable of preforming their job duties. The management will also provide a mentor and training for the staff member to learn appropriate workplace behavior and language.

The parent works at the agency and was aware that the child was missing but no incident report was given or in the child's record — Visit: Complaint Investigation — Action taken: The agency will provide parents with accident/incident reports for all accidents or incidents, even if the parent works at the facility. The agency will file accident and incident reports in all situations to parents even if the parent is an employee.
