The files for Staff Person #1, 2, 3, 4, 5, 6, 9 and 11 do not contain fire safety training conducted within the past 12 months. The files for Staff Person #7, 10 and 12, who have been working at the facility for more than 12 months, do not contain documentation of annual fire safety training (See LIS Code Sheet). Requirement: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Observed three outlets on an extension cord in the 2s and 3s PS Room and one outlet in the Infant Room by the butterflies, which lacked protective receptacle covers. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Contaminated trash in closed plastic-lined receptacle (3270.67(d))

The trash receptacles in the infant room and the gym, which contained soiled tissues, were not lidded. Requirement: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles. Correction: A lidded trash receptacle was placed in the infant room. A new lid was placed on the trash can in the gym. Inspection: Renewal. The first aid kit in the Toddler Back Room lacked tweezers. Requirement: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves. Building Surfaces (3270.76)

Observed a loose, bowed ceiling tile above the number signs in the 2s and 3s PS Room. Observed crumbs and sticky food residue in the cabinet under the snack drawer in the 2s and 3s PS Room. In the rear stairwell, observed dirt and debris on the floor and steps, a broken chair, a ceiling tile and windows propped up against the wall. Observed protruding bolts on the bases of two of the toilets in the SA bathroom. Observed a frayed washer, which creates a sharp edge, in the wall below the clock in the PreK Room. Observed trash and debris in the Toddler Playground. Observed protruding pegs from the structure enclosing the rubber mulch in the PS/SA Playground. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The ceiling tile in the 2's and 3's was replaced. The cabinet in the 2's, 3's was cleaned and crumbs and sticky residue was removed. The rear stairwell was swept and spider webs were removed. The broken chair and the windows were thrown away. Bolts were placed on the protruding toilet screws. The frayed washer from the felt board that was taken down was removed. The hole was repaired. The outdoor playground has not been used as it is still muddy from winter. All trash was removed and any broken toys were thrown away. The pegs that were protruding from the structure enclosing the rubber mulch in the PS/SA playground were pounded down. Inspection: Renewal. Observed flaking plaster between the main child care space and dining area of the PreK Room. Clean and good repair (3270.102(a))

Observed dirt build-up on the steps of the diaper changing table in the Toddler Front Room and on a toy shelf in the 2s and 3s PS Room. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Small Toys and Objects (3270.103)

Observed small beads, less than one inch in diameter, in an unlocked cabinet under the sink in the Toddler Back Room. This presents a potential choking hazard for children who may still be placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.
