Written instructions (3270.133(2))

In the Toddler room observed a container of prescription nystatin cream that was lacking the written instructions for administration from the individual who prescribed the medicine. Requirement: A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable. Correction: The cream was not used and was sent home with the parent. Heat Source (3270.71)

Observed staff person #2 providing child care to a group of 7 school aged children in the facility kitchen where a stove / oven was accessible to the children. Requirement: Hot water pipes and other sources of heat exceeding 110° F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact. Correction: The kitchen will no longer be used for child care. A safety gate is also in place. Inspection: Allocated Unannounced Monitoring. 40 square feet wall-to-wall (3270.61(c))

Observed staff person #2 providing child care to a group of 7 school aged children in the facility kitchen which is not licensed child care space. Requirement: Indoor child care space is measured within permanent stationary partitions or walls. The allowable number of children in a space is determined by dividing the total square feet in a space by 40. Correction: The kitchen area will not be used for childcare at any time. Refrigerator (3270.107)

The refrigerator in the kitchen is lacking a thermometer. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Four chairs being used by school aged children in the gym play space contained fraying fabric on the back of the seats. Requirement: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs. Correction: The chairs were removed and replaced with new chairs. Observed an exposed and protruding bolt at the base of a toilet in the bathrooms off of the gym play space. Requirement: Toilet areas and fixtures shall be cleaned daily and be in good repair. Play space - 65 square feet/child (3270.62(b))

Observed toddler and school aged children using an unlicensed outdoor grassy area behind the facility building as play space. Requirement: Outdoor or indoor play space shall be measured at 65 square feet per child. Correction: The outdoor play space was inspected and is now utilized. Inspection: Renewal. Large muscle space provided (3270.62(a))

Observed the only space licensed as play space for the infants, toddlers, preschoolers and school age children is being used as child care space for the school aged children and is not available for use as play space. Requirement: A facility shall provide outdoor or indoor play space to be used for large muscle activity which includes running, jumping, climbing and riding. Letter to parents (3270.27(e))

The letter provided to parents regarding the facility emergency procedures does not include plans for lock down and does not include accommodations for infants, toddlers and children with chronic medical conditions. Requirement: The operator shall provide to the parent of each enrolled child a letter explaining the emergency procedures. The operator shall also provide to the parent of each enrolled child a letter explaining any subsequent update to the plan. Correction: The terminology for the lockdown and accommodations were added to the letter, was posted, and will be given to parents. Inspection: Renewal. Small Toys and Objects (3270.103)

Observed the textured paint on the floor in the indoor play space is peeling and chipping in 2 observed areas which are accessible to the children in care. Children who are still placing objects in their mouth have access to the chipped paint. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Observed in the older toddler classroom prescription ointment that was not labeled with the name of the child for whom it's use was intended and was lacking the written instructions for administration from the prescriber. The medication log identified that the ointment had been applied to a child in care. Requirement: A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable. The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. Correction: The medication was given back to Mom, she then returned the medication with the correct label.
