Facility self-reported the following incident: Staff #3 and Staff #4 did not maintain supervision of Child #1 at all times. Staff #3 was not physically present with Child #1 on the facility premises. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: The teachers will provide continuous supervision of all children at all times, including in outdoor play areas considered part of the facility premises. Classrooms are arranged in such a way to prevent blind spots. Staff will ensure that no child is ever left unsupervised. The teachers will be assigned specific children for whom they are responsible and will consistently know each child's name and location. Staff will use supervision cards and conduct name to face attendance when crossing thresholds. In the multi-Purpose Room, cert rep observed that the first kit did not contain non-porous gloves, as required. Requirement: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves. Building Surfaces (3270.76)

In the USA, Egypt, Greece, Mexico and Multi-Purpose Rooms, cert rep observed dirty air vents in the ceiling. In the China Room, Cert Rep observed a circular, children's table, stained with sticky, adhesive residue. The table was located on the left side of the room. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: We asked the cleaning company to clean the vents that evening during their visit to the school. China classroom teacher cleaned the sticky, adhesive residue. Inspection: Renewal. Handwashing signs (3270.82(h))

In the Greece, Japan, Mexico and Multi-Purpose Rooms cert rep observed that handwashing signs were not posted above each toilet, as required. In the Australia Room, in the first bathroom, Cert Rep observed that the handwashing sign was not posted, as required. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. Correction: Handwashing signs were posted in missing locations. Clean and good repair (3270.102(a))

In the USA room, cert rep observed several damaged children's books with broken spines, ripped pages and exposed, deteriorating cardboard. These books were accessible to children. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. 2 feet apart (3270.106(f))

In the Italy Room, cert rep observed that cribs in use did not have 2 feet of clearance on three sides, as required. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. In the Greece Room, Cert Rep observed only 4 emergency contact forms while 12 children were present in the care space. In the Multi-Purpose Room, Cert Rep observed that there were no emergency contact forms for any of the children present in the care space. Requirement: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space. Correction: Staff added a folder to the First Aid bag in the multi-purpose room with a copy of each child's emergency contact form. The Emergency Contact forms for children #s 1, 3 and 5 did not have completed parental consent sections for wading and swimming excursions. Requirement: A child's record shall contain signed parental consent for transportation, walking excursions, swimming and wading. Correction: Because we do not offer swimming or wading excursions, those fields were marked "not applicable" for children #s 1, 3 and 5. Inspection: Renewal. During an unannounced monitoring visit, it was observed that multiple cribs were touching while in use in the infant classroom. This was corrected during the inspection. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: Cribs will be shifted apart to leave two feet of space on three sides around each when the cribs are occupied. Inspection: Unannounced Monitoring. Correction: Classroom teachers received formal documentation. Directors implemented enhanced monitoring of classroom transitions---from departure to return---beginning Tuesday, 9/15 through Tuesday, 10/6. This measure was put in place to ensure all procedures are followed consistently and safely and to give feedback on how to help the classroom teachers if they had questions. Documentation of the clearances on file for staff member 3, did not include out of state clearances and was observed working with children. Documentation of the state police clearance, child abuse clearance and FBI clearance on file for staff member 1 were updated more then 60 months of the original date. The state police clearance expired on 6/18/24 and was updated on 7/16/24. The child abuse clearance expired on 6/18/24 and was updated on 7/16/24 and the FBI clearance expired on 7/03/24 and was updated on 7/18/24. The out of state clearances for staff member 3 was corrected on 4/29/25. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person #3 -- may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #3--- may not work in a child care position at the facility. Correction: Teacher 1 requested required out of state clearances and did not work with children until they were received. Small Toys and Objects (3270.103)

During the time of inspection, it was observed that science sensory bottles with small beads, small stars and uncooked pasta pieces; all less than 1 inch in diameter were all able to be opened causing a possible choking hazard in the Japan classroom. All sensory bottles were removed during the inspection. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. During the time of inspection, it was observed that a basket with dinosaurs it in was visibly dirty in the Egypt classroom and a broken basket with wooden play people in the China classroom. Both were corrected during the inspection. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The dinosaur basket in the Egypt classroom was moved out of reach of children. It was then cleaned of dirt during nap time. During the time of inspection, it was observed that there was chipped paint behind the door in the USA classroom. There was chipped paint behind the diaper trashcan in the Egypt classroom. There was chipped paint under the window by the door and chipped plaster behind the bookshelf in the China classroom. All of the chipped paint and plaster were corrected during the inspection. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. There were cobwebs between the door leading the to the playground and the wall in the Greece classroom. During the time of an unannounced monitoring visit, it was observed that sunscreen was within reach of children in the Australia classroom. This was corrected during the inspection. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.
