On 1/7/2026 child #1 (see LIS coded sheet) sustained an injury to their mouth area. An injury report was not given to the parent on the day of the incident. Requirement: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file. Correction: If parent is not available to sign/receive the incident report the day the incident occurred due to someone else picking up the child, I will put that the parent was not available to sign/receive with my signature. The report will be sent through Brightwheel and text letting the parents know that an incident took place, so they are notified on the day the incident occurred. The original hard copy will be provided to the parent when they return to the center. In this case, the parent/child did not return to the center, so the incident report was sent by mail. Handwashing signs (3270.82(h))

It was observed on 8/20/2025 in the upstairs bathroom by the Pre-K room, a hand washing sign is not posted at each toilet and sink. (CORRECTED ON SITE) Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. It was observed on 8/20/2025 in the Young Toddler room a written plan of daily activities is not posted in the group space. (CORRECTED ON SITE) Requirement: The written plan of daily activities shall be posted in the group space. It was observed on 8/20/2025 in the 2's classroom, a written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency was not displayed conspicuously in the child care space. (CORRECTED ON SITE) Requirement: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children. Correction: During the inspection a written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency was posted in the 2's classroom. Disposable diapers (3270.135(a)(3))

It was observed on 8/20/2025 in the 2's classroom, soiled diapers are not being discarded in a hands-free covered can. Staff indicated that they use their hand to open a cabinet door on the changing table to discard a soiled diaper. (CORRECTED ON SITE) Requirement: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. Correction: During the inspection staff members pulled out a plastic-lined, hands-free lidded trash can from the bathroom and placed it beside the changing table prior to changing diapers. 12 months prior to service and every 24 months thereafter/Mantoux TB (3270.151(a)/3270.151(c)(2))

The record for Staff Person #1 (DOH see LIS code sheet) includes the results of a tuberculosis screening by the Mantoux method dated 2/23/2022, which is more than 12 months prior to providing initial service in a child care setting and therefore is not valid. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department. Correction: Sent copy of health assessment to staff person #1. Staff person #1 took care of that as soon as I sent her a copy of the health assessment showing her TB was not marked. Employee #1 was unable to work until documentation was completed by her physician and turned into the director. TB was completed on 8/20/2025. Inspection: Renewal. Staff assigned to specific children (3270.113(a)(1))

It was observed on 6/11/2025 at approximately 7:30 am four (4) staff members were present in one classroom with a total of fourteen (14) children (4 infants, 5 toddlers, and 5 preschoolers). Staff members #1, #2, #3, and #4 were not assigned the responsibility for supervision of specific children. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: All staff will go the correct rooms in the morning. Name to face cards are now being used. The staff will know the names and ages of the assigned children in their group. Clean, good repair, proper size (3270.104(a))

It was observed on 6/11/2025 at approximately 7:35am a total of four (4) infants were seated in a car seat, bouncer, or a low chair with a tray and sitting in front of a television screen with a movie playing. Ten other children of a mixed age group (5 toddler and 5 preschoolers) were also in the room. The car seat was not being utilized for the intended purpose of protecting a child from injury or death while being transported in a vehicle. Requirement: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs. Correction: Drop off for supervision, no children in the facility will no longer be kept in car seats or unsafe furniture. The infants will be separated from older children to ensure this does not happen again. As soon as children get here, they will be taken out of car seats. All car seats will be placed in the parent corner. Name to face cards are now being used. No movies will be played. Materials will be out for the children. Tv's will be covered as music is played. Promote development (3270.111(c))

It was observed on 6/11/2025 at approximately 7:35am a total of four (4) infants were seated in a car seat, bouncer, or a low chair with a tray and sitting in front of a television screen with a movie playing. Ten other children of a mixed age group (5 toddler and 5 preschoolers) were also in the room watching the movie. This does not promote the development of skills, social competence and self-esteem. The children were not provided a choice of activities that respect personal privacy, lifestyle and cultural background. Requirement: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background. Correction: Staff will ensure that Tv time/screen time will only happen at transition times. All staff are aware of developmentally appropriate activities to promote the development of children. When Children enter classroom materials, table toys, writing table, coloring pages children will have access to the centers. Restraints prohibited (3270.113(e))

During an interview on 3/7/2025 Staff person #1 admitted to placing child #1 (DOB see LIS code sheet) in a toddler sized chair with a snap on tray to keep child #1 from kicking and hurting other children and staff members. Staff person #1 stated that child #1 was in the chair for several minutes until the child's parent arrived to pick up the child from the facility. The incident occurred on 2/26/2025. Staff person #1 also admitted to previously holding child #1 on their lap and placing their leg around the child in an attempt to get child #1 to calm down. Staff person #1 stated that while child #1 was being held on their lap, child #1 would "thrash around and try to head butt" staff person #1. Requirement: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs. The legal entity must arrange for all facility staff responsible for the supervision of children to participate in a minimum of 2 hours of in person training provided by ELRC staff regarding appropriate discipline practices for children with challenging behaviors. Correction: A facility person may not restrain a child to restrict a child's movement or by enclosing the child in a confined space. In this facility we will no longer place children in a toddler size chairs with snap on trays or holding a child and wrapping our legs around them. All of the facility staff in the center will participate in a minimum of a 2 hour in person training provided by a ELRC staff regarding appropriate discipline practices for children with challenging behaviors.
