The certificate of compliance posted in the facility expired 12/20/2025. Requirement: The facility's current certificate of compliance shall be posted in a conspicuous location used by parents. The operator shall provide the parent of each child enrolled with information on how to access the regulations in this chapter electronically and with instructions for contacting the appropriate regional child care office. Observed in the preschool room on the wall behind the whiteboard crumbling cement block and paint. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: Teachers will plan ongoing checks of the wall surface to ensure that any bubbled, chipped or peeling areas are scrapped, cleaned and painted when repair is needed or items are taken down from the wall. Protective Outlet Covers 5 yrs. or less (3270.65)

Observed in the preschool room on the wall by the whiteboard an electrical outlet that was lacking a protective cover and was accessible to children age 5 and under. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Correction: Teachers will plan to do a walk through of the room in the morning before children arrive to check outlet covers and replace any that are missing. Observed in the preschool classroom a power strip with 5 electrical outlets that were lacking protective covers. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. The fire system test log identified alarm tests exceeded 30 days between tests from 11/24/23 to 12/26/23 and again from 3/25/24 to 4/25/24. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. The fire drill log identified that fire drills exceeded 60 days between drills from 9/1/23 to 11/1/23 and again from 2/26/24 to 5/29/24. Requirement: The Director or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that fire drills are conducted at least once every 60 days. The file for staff person #1 contains a current health assessment dated greater than 24 months after the previous health assessment. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. Observed 2 ceiling tiles in the preschool room to be stained showing evidence of a water leak. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
