While completing an allocated unannounced monitoring visit, it was observed that a basket with dramatic play items was broke in the young toddler classroom and a basket with rattles was broke in the infant classroom. During the time of inspection, it was observed that there was a hole in the wall in the indoor gross motor space and multiple plastic barriers that surround the outside playground were cracked and broken. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Baskets containing magnatiles and dramatic play were broken in the young toddler classroom. The climbing equipment in the young toddler classroom was visibly dirty.
