During naptime, in the Younger Preschool Room, Staff Person #1 identified 5 children in their primary care group and Staff Person #2 identified 3 children in their primary care group. There were 2 classes combined for naptime with a total of 14 children. Neither staff person included the other staff person's (on break) 6 children as part of their primary care groups. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: Staff were given tags to identify each child and who is in each teacher's core group. This strategy was gone over with each classroom team. Inspection: Renewal. A Medication Log documented that Child #1's medication was administered on 1/22/26, however the staff person had not included the time that the medication was administered. Outdoor Activity (3270.114)
Children in the Prekdg class were not regularly taken outdoors daily. Staff indicated that they take children out in the afternoons, but not always in the mornings. While children go to the gym some mornings for gross motor play, there are many children who are picked up before children go outside to play on the playground (4:15 or 4:30), so those children do not go outdoors daily, weather permitting. Class schedules include a morning and an afternoon outdoor play time. Requirement: Weather permitting, children shall be taken out doors daily. Correction: Director and Assistant Director will install a traffic light system in the hallway that indicates if the weather allows the children to go outside. Two Magic Erasers were located by the paper towels that the children use for handwashing. They were not labeled and were accessible to the children in the School Age Room/Gym. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces. Correction: The magic erasers were put into a sandwich bag, labeled with the contents, and made inaccessible to the children. Inspection: Renewal. Several hexagon rubber surfacing pieces on the Toddler Playground under the climber and at the entryway, were loose or missing. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: All pavers were placed back into the appropriate spot during the time of the inspection. Dirt was filled in around each paver to secure them and prevent them from moving.
