    During the renewal inspection, the file for Staff #1 contained an NSOR certificate received 11 weeks after their first day caring for children, incomplete out-of-state clearances and the clearances had not been submitted to OCDEL for a required review. The file for Staff #4 contained an NSOR received 7 weeks after their first day caring for children and a state police clearance in "request still pending status". Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 1 may not work in a childcare position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 may not work in a childcare position at the facility. Correction: Staff #1 has current NSOR as of 4/27/26. Staff #1 has all PA and out of state clearances as of 5/19/26. Staff #1 will not work in tis center until cleared by DHS. Staff #4 has a current NSOR as of 4/27/26. Staff #4 is no longer working at our childcare center as of 5/15/26. Inspection: Renewal.     During the renewal inspection, there were seven staff employed with the following five staff qualifying as aides: Staff #1, #2, #3, #4, and #5. Requirement: At least 50% of the staff persons in a facility shall be individuals qualified and functioning as directors, group supervisors and assistant group supervisors. No more than 50% of the staff persons may be of aide qualification level. Correction: Staff #1 is not working with children at our center until cleared by DHS. Staff #2 and #5 have verified childcare hours and are now AGS status; no longer aides. Staff #3 has verified childcare hours and is a volunteer until she graduates from high school on 6/15/26.     During the renewal inspection, twenty-eight exposed/uncovered metal bolts were observed on the outdoor playground fence posing a visible hazard. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Black plastic covers were purchased to cover all of the exposed screws on our outdoor fence. They are now in place. Inspection: Renewal.
