Requirement: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space. Correction: A copy of all emergency contact forms was immediately added to the first aid kit that was taken to the play yard to ensure that they would be present in the play yard at all times. Emergency contact form for child #1 was missing the parent work address. Emergency contact form for child #2 was missing the mother's home and work address. Requirement: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent. Correction: Upon notification of the missing parent work address for child #1 and missing home and work addresses of the mother of child #2 the forms were pulled and the parents were asked to complete them upon picking up there children from care on the same day. Staff number 1 was observed to be working without a complete State Police clearance. State Police clearance that listed the results as Pending, which is not a completed clearance. Corrected on site by obtaining the complete clearance. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Correction: Upon notice of PENDING on the criminal, State Police, clearance the site was visited immediately and a clearance with the proper date and results was obtained and retained in the staff's file Inspection: Renewal. There was no first aid kit present with children while on the playground. Requirement: A first-aid kit shall be in a child care space. Correction: Upon notice that a first aid kit needed to be in every space including outdoor areas, a new first aid kit with supplies was acquired and placed in the outdoor area so that it would be present at all times. No peeling paint or plaster (3270.77(a))

In the outside playspace, a wall opposite of the entrance was observed to have peeling paint accessible to children. Peeled paint chips were observed on the ground in the area Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: Upon being notified that the paint was peeling on the wall of the play yard a phone call was placed to the building maintenance department and they were shown the area that needs to be repaired. Clean and good repair (3270.102(a))

In the backroom, toy bins were observed to be greasy and dirty. A Cocomelon steering wheel was also observed to be greasy and dirty. In the infant area a stand and play with blue legs, a yellow and green pop up toy, and a teal sit and stand toy were observed to be greasy and dirty. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: Upon notification of stickiness and dirt on toys, they were pulled and inspected. IT was determined that the new disinfectant spray was causing sticky build up and a new disinfectant spray was purchased. All toys including the ones mentioned in the violation and all other toys in the center were cleaned with a bleach and water solution and the new disinfectant spray was implemented immediately. A written plan of daily activities was missing from the back room. Requirement: The written plan of daily activities shall be posted in the group space. Correction: Upon notification that the daily schedule was missing in the back room, a new one was printed and placed in a conspicuous location where anyone can see it. Inspection: Renewal. Building Surfaces (3270.76)

Window sills in the back child care space were observed to be dusty Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: All windowsills in the back child care space were cleaned upon notification that they were observed to be dusty. Supervised at all times (3270.113(a))

In the back room, a staff was observed changing a child's diaper. During the diaper change, the staff could not actively supervise infants across the room in a separate area due to the placement of the changing table. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: A plan was immediately discussed to ensure that when groups of children and staff are outside and one staff member is left with infants in the back play space to change diapers and supervise children, staff will use a bell to ring for assistance to be able to change diapers without compromising supervision. Any children coming in from outside will be accompanied to the other room as well, so that supervision standards are being met at all times. Inspection: Renewal. Staff #1 was observed to have an initial health assessment dated 8/30/24, which was completed after initial employment. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. During the smoke detection testing, both alarms were observed to operate independently. Both alarms were operable but were not interconnected as required per Act 62. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Correction: The center owner will procure new smoke detectors that do not only state to be interconnected, but will ensure that they are interconnected. Once purchased the new smoke detectors will be installed to maintain compliance with standards and will be tested to to be sure that if one goes off they will all go off. Inspection: Renewal.
