    During an activity, staff #1 was observed to grab a child, lift the child up, flip the child upside down and then place the child's face in a painting. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: On the date of the incident (10/8/2025) Staff #1 was sent home and suspended from working in any classroom int he center, until the investigation was completed. The center director self-reported the concerning behavior to DHS and the parent was notified. CYS was contacted as well. Inspection: Complaints- Legal Location.     In the Preschool room, 15 children were observed were observed in care with 3 staff present. Staff did not have the group broken up with specific children assigned to their group. On the playground, 3 staff were observed with 14 school aged children. One staff listed their group as 5 children. When the other 2 teachers were asked, the teacher did not have assigned care groups. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: All classrooms will have pictures of the children in their primary groups and be responsible when asked of sharing the names of the children in their groups. Administration is going to periodically visit rooms to ask staff to name their groups and how many children are present. We will continue to use white boards as well for daily attendance tracking and head counts. The teachers are consistently aware of how many children are present in their classrooms when asked. In addition, during our staff meeting on 6/17/25, staff were retrained on primary care groups. Inspection: Allocated Unannounced Monitoring.
