The fire extinguisher in the kitchen area was inspected in January 2025 when it was provided to the operator by the local fire department; however, at the time of renewal inspection, the gauge was not reading in the operable (green) range. The gauge showed that the fire extinguisher was under-charged. Clean and good repair (3290.102(a))

In the childcare space, a green plastic basket containing toys was observed to be cracked, causing a sharp edge. In the childcare space, the white cube shelf beneath the facility's posting board was observed to be in poor repair. Specifically, the shelf wobbled when touched. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: In the childcare space , the green basket has been removed from the area and thrown away.The white cube shelf has also been removed from the facility area and replaced with a new shelf holding bins of toys. The files for children #1 and 2 lack the child's immunized status according to the recommendations of the ACIP or an exemption letter written by the parent. Requirement: : Child(ren) _ #1 and 2 must be dismissed from care by close of business on date of provider notification and may only return to care when the immunization record is updated or when a parent provides a written, signed exemption letter. A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification.
