During an unannounced inspection on 5/5/2026 from approximately 4:00pm to 5:40pm staff person #1, the director, was unable to provide certification representatives with the whereabouts of staff persons #2 and #3 and a group of 11 preschool and school aged children. During a follow-up visit on 5/7/2026 staff person #2 stated that the group had been on a walk and returned to the facility at ~7:00pm and was unable to send or receive communication with the facility due to not having an operable cell phone. The facility did not provide certification representatives immediate access to the children during the unannounced inspection. Requirement: A staff person shall provide to agents of the Department immediate access to the facility, the children and the files and records. On 5/5/2026 staff persons #4 and #5 were observed supervising a group of 2 young toddlers and 2 infants. Neither staff person #4 nor staff person #5 knew the name of one of the infants who they stated was newly enrolled. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Observed a torn pad on the changing table in the bathroom exposing the foam interior which was accessible to the infants and toddlers being changed on the pad. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.
