According to staff, infants in the Frogs classroom are not going outside daily weather permitting. Requirement: Weather permitting, children shall be taken out doors daily. Correction: The frog classroom will be taking students outside on a daily schedule, weather permitting, based on feeding and napping schedules, as well as ratio. Students may be taken outside throughout the day with the help of an assistant or float, in order to carry each of the babies. Inspection: Renewal. Observed a box containing small Styrofoam bowls in the storage closet and according to staff, small Styrofoam bowls are used by preschoolers in care. Requirement: Styrofoam cups and plates may not be used. Correction: The director removed the Styrofoam bowls from the classroom and the storage closet. Inspection: Renewal. Small Toys and Objects (3270.103)

Observed on the floor in the Zebra's classroom where older toddlers were receiving care, a small, less than 1 inch piece of broken crayon accessible to toddlers still placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: The broken crayon was thrown away, the staff were asked to go through their remaining crayons and told to throw out any small broken pieces. Inspection: Renewal. Clean and good repair (3270.102(a))

Observed on the climbing equipment on the small preschool playground, a small area on the lower platform with damage to the protective vinyl coating. Observed in the elephant classroom, a small wooden bookshelf with loose screws effecting the shelf's stability. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The lower platform on the small preschool playground was temporarily covered with electrical tape. The rubber replacement has been ordered and will be fixed by 6/30/26. Building Surfaces (3270.76)

Observed on the fence on the large preschool playground an approximately two-inch circular crack with sharp edges. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The fence piece was initially taped over the day of the inspection, maintenance has since flipped the piece over so that the side with the hole is no longer exposed to children, while they await a replacement piece.
