Staff #4's most recent fire safety training was dated 8/21/24 which was more than 12 months ago. Requirement: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Supervised at all times (3270.113(a))

Per staff statements, Staff #1 placed Child #1 in a low highchair that did not have straps and walked away to get the tray. Child #1 fell forward and hit their head. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: All children in the facility are supervised at all times. In the Butterfly Room(103) the first aid kit was missing sterile gauze. In the Fox room(105) there was a protective outlet cover missing from a power strip near the door. In the Deer Room(113) there were 2 protective outlet covers missing from a power strip near the refrigerator. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. In the outside preschool play space, the outside book bag which contained sunscreen, that stated keep out of reach of children, was sitting on a wooden box accessible to children. In the Fox Room (105) the outside bag was hanging on a low hook with sunscreen, that stated keep out of reach of children, was in an outside pocket accessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. At approximately 115pm in the Porcupine Room(112) it was observed that 8 children were resting on cots that were not at least 2 feet on 3 sides of the rest equipment. Correction: The cots in the Porcupine classroom have now obtained 2 feet of space on 3 sides by moving the cots locations, when in use. Inspection: Renewal. In the coyote room with older toddler age children were foam blocks with teeth marks and missing pieces of the block were bitten. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.
